摘  要:目的探讨溴隐亭(BRC)对泌乳素腺瘤临床疗效及细胞凋亡的影响。方法回顾性分析59例泌乳素腺瘤患者的临床资料,其中药物治疗组49例,非药物治疗组10例,比较药物治疗前后肿瘤直径和泌乳素(PRL)水平变化;再选取部分患者病理标本,采用TUNNEL法比较药物治疗组(20例)与非药物治疗组(10例)的凋亡指数差异。结果药物治疗组患者的肿瘤直径和PRL水平均有不同程度的降低。微腺瘤服药4~8周和9~52周的药物治疗有效率为33.33%vs 66.7%(P=0.31),大腺瘤服药4~8周和9~52周的药物治疗有效率为15.38%vs 57.14%(P=0.03)。药物治疗组中50%的标本可以检测到细胞凋亡,非药物治疗组为40%,药物治疗组细胞凋亡指数与非药物治疗组比较有显著性差异(1.88%vs0.95%,P=0.000 5)。结论 BRC治疗泌乳素腺瘤是安全有效的。BRC可以诱导泌乳素腺瘤细胞凋亡;诱导的细胞凋亡在药物治疗泌乳素腺瘤中起着一定的作用。Objective To explore the influence of bromocriptine (BRC)on clinical outcome and apoptosis of prolactinoma, and discuss the possible mechanism involved in it.Methods Retrospective analysis of 59 cases with diagnosis of prolactinoma ( including 49 cases treated with BRC ranging from 4 week to 52 weeks and 10 cases without medical treatment). Clinical data ineluding tumor size, prolactin (PRL) levels before BRC and sequentially thirty cases of tumor specimens examined consisted of two groups: group I (10 cases) received no medication and group II (20 cases) continued bromocriptine treatment until the operation. The labeling of nuclear DNA fragmentation according to the TUNEL method was considered as an index of apoptosis (AI). PRL was measured by radioimmunoassay (RIA). Results All treated patients had reduced serum PRL levels and tumor size. The effective rate between 4- 8 weeks and 8~ 52weeks in microadenma were 33.33% and 66.7% (P= 0.751), and in macroadenoma, the effective rate of 15.38% and 57.14% (P = 0. 041 ). Apoptosis could be detected in both groups. There was significant difference in apoptosis between group with BRC and group without medical treatment ( 1.88% vs. 0. 95%, P = 0. 01 ). Conclusions Bromocriptine was safe and effective in controlling prolactinomas. This is the first moderate-sized group of patients in whom apoptosis induced by bromocriptine therapy( ranging from 4 to 52 weeks).The findings seem to indicate that the reduction in size of prolactinomas by bromocriptine treatment results from the reduction in size of individual tumor cell as well as from cell loss secondary to apoptosis induced by BRC.
