垂体后叶素局部注射在腹腔镜卵巢巧克力囊肿剥除术中的应用

摘　　要：目的探讨在全麻腹腔镜卵巢巧克力囊肿剥除术中,卵巢囊肿壁内注射垂体后叶素对患者血流动力学、囊肿剥除时间、手术时长及失血量的影响。方法全麻下拟择期行腹腔镜单侧单个卵巢巧克力囊肿剥除术患者,ASAⅠ~Ⅱ级,随机分入A组（卵巢囊肿壁内注射稀释的垂体后叶素10 ml,n=28）、N组（卵巢囊肿壁内注射生理盐水10 ml,n=27）和C组（常规剥除组,n=30）。记录卵巢囊肿壁内局部注射前即刻（T_1）,局部注射后即刻（T_2）、30 s（T_3）、1 min（T_4）及2 min（T_5）的MAP和HR,记录囊肿剥除时间（T_b）、手术时长（T_s）和出血量。结果三组患者一般统计学资料差异无统计学意义。组内比较,与T_1相比,各时间点的MAP、HR无显著差异（P〉0.05）。组间比较,各对应时点的MAP、HR差异亦无统计学意义（P〉0.05）。A组和N组的T_b短于C组（P〈0.05）,A组的T_s短于N组和C组（P〈0.05）。在术中失血量方面,A组少于N组和C组（P〈0.05）。结论卵巢囊肿壁内注射垂体后叶素不影响患者血流动力学,能有效缩短囊肿剥除时间、手术时长,减少术中出血量,可以安全的用于腹腔镜下卵巢巧克力囊肿剥除术。Objective To investigate effects of diluted pituitrin, treated with local injection, in laparoscopic cystectomy of ovarian endometrioma with general anesthesia. Methods Eighty-five patients, ASA I - II, scheduled for laparoscopic cystectomy of ovarian endometrioma with general anesthesia, were randomized into three groups： group A （received diluted pituitrin 10ml, n = 28）, group N （received 10ml of normal saline, n=27）and group C （no treatment n=30）. Heart rate （HR） and Mean arterial pressure （MAP） were measured before use of diluted pituitrin （ T~ ）, immediately after use （ T2 ）, 30s （ T3 ）, lmin （ T4 ） ,2min （ T5 ）. The time of ovarian cystectomy （Tb） , surgery time （T~）, and blood loss of surgery （ml） were also noted. Results There were no significant differences in general information and MAP, HR at every time-point between the three groups. Compared with MAP and HR at T1, there were also no difference at T2-T5 in every group. Times of ovarian eystectomy（Tb） in group A and group N were shorter than group C （P〈0.05）. Surgery time （Ts） and blood loss （ml） ： group A were shorter than group N and C （ P 〈 0. 05 ）. Conclusions Topical use of diluted pituitrin in laparoscopic cystectomy of ovarian endometrioma undergoing general anesthesia has less impact of hemodynamic, which can decrease times of ovarian cystectomy, surgery time and blood loss.
