X线立体定向放射治疗同步口服替莫唑胺化疗对脑转移瘤的疗效观察

摘　　要：目的观察X线立体定向放射治疗同步口服替莫唑胺化疗与单纯X线立体定向放射治疗脑转移瘤的临床疗效及不良反应。方法临床纳入脑转移瘤患者47例,根据治疗方案的不同分为研究组与对照组。研究组进行X线立体定向放射治疗同步口服替莫唑胺化疗,对照组仅进行单纯X线立体定向放射治疗。观察2组临床疗效、肿瘤局部复发率、中位复发时间、中位生存时间等,同时观察治疗期间2组不良反应发生情况。结果研究组治疗有效率92.86%,对照组为68.42%,差异有统计学意义(P<0.05);研究组肿瘤局部复发率28.57%,对照组为31.58%,差异无统计学意义(P>0.05);研究组中位复发时间、中位生存时间分别为12.2个月、13.7个月,对照组分别为9.6个月、13.7个月,差异有统计学意义(P<0.05);2组治疗期间不良反应发生率差异无统计学意义(P>0.05)。结论 X线立体定向放射治疗同步口服替莫唑胺化疗治疗脑转移瘤的近期临床疗效优于单纯放疗,能够有效延缓中位复发时间,延长中位生存期,且不良反应发生率未见明显提高。Objective To observe the curative effect and adverse effect of X-ray stereotactic radiotherapy combined with Temozolomide and X-ray stereotactic radiotherapy in the treatment of metastatic encephaloma .Methods Forty-seven patients with metastatic encephaloma were selected and divided into study group and control group according to treatment perscription . The study group adopted X-ray stereotactic radiotherapy combined with Temozolomide ;the control group adopted X-ray ster‐eotactic radiotherapy .After treatment ,the clinical effect ,local recurrence rate ,median recurrence time and median survival time as well as the incidence of toxic and adverse effect in two groups were observed .Results The effective rate of study group （92.86% ）was higher than that of control group（68.42% ）（P〈 0.05）;the local recurrence rate of study group and control group was 28.57% and 31.58% （P〉0.05）;the median recurrence time and median survival time for study group was 12.2 months and 13.7 months while the time in control group was 9.6 months and 13.7 months（P〈0.05）;the incidence rate of toxic and adverse effect in two groups was not significantly different （P〉0.05） .Conclusion For the metastatic encephaloma , X-ray stereotactic radiotherapy combined with Temozolomide has a better short-term effect than X-ray stereotactic radiothera‐py ,and can effectively delay the median recurrence time ,and extend the median survival time .The incidence rate of adverse effect is not greatly increased .
