血清CA125在子宫内膜异位症复发诊断的价值

摘　　要：目的分析血清CA125在子宫内膜异位症（EMs）保守治疗术后复发的临床意义。方法回顾性分析135例接受保守手术治疗的EMs患者，分析术前、术后血清CA125水平与术后复发的关系。结果血清CA125对诊断EMs的灵敏度在不同分期患者有显著差异（P〈0．05），分别为Ⅰ期7．1％，Ⅱ期36．2％，Ⅲ期80．4％，Ⅳ期88．2％。复发组患者术前CA125明显高于未复发组患者，差异有统计学意义（P〈0．05）。术后血清CA125可以辅助诊断EMs复发，将ROC曲线临界值定为38kU／L，其敏感度为76．1％，特异度为89．9％，阳性预测值为79．6％，阴性预测值为87．9％。结论EMs患者血清CA125水平与疾病严重程度相关，分期越高，血清CA125阳性率越高。术前血清CA125水平高是EMs复发的危险因素。术后血清CA125检查可以作为EMs复发的一种监测手段。Objective To evaluate the diagnostic value of serum CA125 for the recurrence of endometriosis. Methods A regression analysis was performed at 135 patients who received endometriosis conservative surgery to evaluate the relationship between serum CA125 and the recurrence of endometriosis. Results The sensitivity of the serum CA125 for stage Ⅰ, Ⅱ, Ⅲ and Ⅳ were 7.1%, 36.2%, 80.4% and 88.2%. The preoperative serum CA125 of recurrent patients were significantly higher than those of non-recurrence. Receiver operating characteristic curve analysis with post-operation serum CA125 cutoff point of 38 kU/L showed sensitivity of 76.1%, the specificity of 89.9%, the positive predictive value of 79.6%, and the negative of predictive value of 87.9%. Conclusion The level of CA125 is associated with the severity of endometriosis. A high serum CA125 level is a risk factor for the recurrence of endometriosis. Monitoring post-operation serum CA125 level is a simple and easy way to diagnosis the recurrence of endometriosis.
