非接触桥接钢板倒置用于股骨近端良性骨肿瘤术后内固定

摘　　要：目的使用倒置的股骨非接触桥接钢板用于股骨近端良性肿瘤的内固定,评价治疗效果。方法回顾性分析长海医院骨科2011年1月至2015年1月收治且符合研究要求的患者44例,分为A、B两组。肿瘤刮除植骨后,A组26例患者使用股骨远端非接触桥接钢板倒置后作为内固定,B组18例患者佩戴外固定支具。比较两组患者术后的不良事件发生率、患肢开始负重时间、髋关节Harris功能评分等。结果比较两组患者术后植骨相关不良事件的发生率,P>0.05。A组患者没有发现内固定松动、断裂及螺钉退出。B组1例患者术后出现植骨部位骨折。A组患者患肢开始负重时间早于B组患者,P<0.001。A组患者术后1个月、3个月的Harris评分高于B组,P值分别为0.009和0.007,差异有统计学意义。结论非接触桥接钢板倒置后用于股骨近端良性肿瘤刮除后的内固定,效果满意,能够使患者早期下地进行功能锻炼,并促进髋关节功能的恢复。Objective To evaluate the treatment efficacy of proximal femoral benign tumor with reversed non-con-tact bridging plates.Methods We retrospectively analyzed the 44 patients who suffered benign tumor in proximal fe-mur and received treatment in the department of orthopaedics of Changhai hospital.After the curettage of tumor lesion and bone grafting,26 patients in Group A received the internal fixation with reversed non-contact bridging plates,while 18 patients in Group B received the external fixation with braces.The adverse events rate,the time of weight loading af-ter operation,and Harris hip scores （one month,three months and half a year after operation respectively）between the two groups were compared.Results There was no loosening or breakage of internal fixation in Group A during the fol-low-up,while one patient in Group B suffered fracture in the bone grafting location.The time of weight loading after op-eration in Group A was shorter than that in Group B,P〈0.001.The Harris hip scores of one month and three months after operation in Group A were higher than those in Group B.Differences were statistically significant as the P values were 0.009 and 0.007,respectively.Conclusion Reversed non-contact bridging plates have a good result in the treat-ment of the proximal femoral benign tumor.The plate helps to shorten the time before weight loading after operation and promote the functional recovery of hip
