负压吸引器与抗反流引流袋在胃癌患者胃肠减压中的应用效果

摘　　要：目的评价负压吸引器与抗反流引流袋在胃癌患者胃肠减压中的应用效果。方法 2015年6-12月,便利抽样法选取在南京大学医学院附属鼓楼医院普通外科行根治性远端胃大部切除术(毕Ⅱ式)的胃癌患者120例为研究对象。所有患者术后除留置胃管外均行加速康复外科护理。根据外接引流不同分为外接负压吸引器组和抗反流引流袋组,每组60例。比较两组患者术后排气排便时间、腹胀或其他腹部不适及吻合口瘘的发生率、平均住院时间和术后患者满意率等。结果和胃管外接负压吸引器组相比,外接抗反流引流袋组术后腹部不适包括恶心呕吐、腹胀的发生率较低,首次排气排便时间、平均住院日相对缩短,差异均有统计学意义(均P<0.05)。外接抗反流引流袋组的患者满意率高于外接负压吸引器组,差异有统计学意义(P<0.05)。结论对于行根治性远端胃大部切除术(毕Ⅱ式)患者,术后采用胃管外接抗反流引流袋能显著减少患者腹部不适,提高护理质量。Objective To evaluate the application effects of vacuum extractor and anti-reflux drainage bag in patients with gastric carcinoma.Methods A total of 120 cases of patients with gastric carcinoma under subtotal gastrectomy（Billroth Ⅱ） were randomly selected and divided into two groups according to different means of gas- trointestinal decompression, vacuum extractor or anti-reflux drainage bag.Each group had 60 patients and all of them received fast track surgery nursing.The exhaust defecation time, abdominal distension or the other abdominal discomfort, the incidence rate of anastomotic leakage, the average hospitalization time, and the satisfaction level of patients were compared between the two groups. Results After the application of anti-reflux drainage bag, the incidence rate of abdominal distension or the other abdominal discomfort was obviously lower and exhaust defec ation time and average hospitalization time were shorter（all P 〈0.05）.The satisfaction level in anti-reflux group was higher compared with patients in the vacuum extractor group （P 〈 0. 05 ）. Conclusions The application of anti-reflux drainage bag in patients under BillrothⅡgastroenterostomy can reduce the post- operative abdominal discomfort and improve the nursing quality.
