卵巢交界性肿瘤保留生育功能术后体外受精-胚胎移植9例临床分析

摘　　要：目的探讨卵巢交界性肿瘤经保守性手术治疗后进行体外受精-胚胎移植(IVF-ET)治疗的安全性及有效性。方法回顾性分析2009年1月至2014年12月因不孕症就诊、既往卵巢交界性肿瘤行保留生殖功能手术的9例患者(肿瘤组)的临床资料,与同期因不孕症就诊无卵巢交界性肿瘤而行附件切除术病史的9例患者(对照组)按照同质配对方法进行配对,比较两组间的IVF相关指标及肿瘤复发情况。结果 9例肿瘤组和对照组患者生化妊娠分别为4例和5例(P=0.637),临床妊娠分别为3例和5例(P=0.343),活产分别为2例和5例(P=0.147),两组均无流产、早产发生。术后随访18个月,9例肿瘤组患者中1例复发,该患者术后经IVF-ET助孕足月产后半年(术后18个月)时右侧卵巢肿瘤复发。结论卵巢交界性肿瘤实行保留生育功能手术对后续不孕症行体外受精鲜胚移植妊娠结局无显著不良影响。Objective To study the effect and security of IVF（in virto fertilization） treatment for women underwent fertility sparing-surgicalmanagement due to borderline ovaries tumors.Methods Retrospective analysis of 9 cases,namely tumor group,those patients came to our hospital because of infertility and undergone fertility sparing-surgical management for borderline ovaries tumors,compared to patients,namely control group,those diagnosed with infertility and undergonesalpingooophorectomyswithout tumors from Jan.2009 to Dec.2014.Analyzed two groups relevant items of IVF and follow-up studies on tumor group in order to discuss the effect and security of IVF treatment.Results According to two groups IVF outcomes：biochemical pregnancy rate was 44% vs 56%（P=0.637）,clinical pregnancy rate was 33% vs 56%（P=0.343）,live birth rate was 22% vs 56%（P=0.147）.Two groups have no miscarriage and premature delivery.Based on follow-up presently,only one recurrence case in 9 cases carried out radical surgery management.Conclusion Infertility patients who after fertility sparingsurgical management on borderline ovaries tumors had no abnormal IVF outcomes and high recurrence condition.
