摘  要:目的探讨不同牙体预备和修复方法对重度楔状缺损牙体抗折性能的影响,为临床修复治疗提供理论依据。方法将64颗离体下颌第一前磨牙按不同的牙体预备和修复方法分为3组,正常组(A组)、重度缺损组(B组)、重度缺损治疗组(C组)。将C组按照颊侧楔状缺损方悬突牙体去除与否分为2组,保留悬突组(C1组)、去除悬突组(C2组),每组按照修复方式分为3个亚组,纤维桩树脂修复组(1组)、纤维桩全冠修复组(2组)、铸造桩全冠修复组(3组),即C1-1组、C1-2组、C1-3组、C2-1组、C2-2组、C2-3组。共8组,每组8颗,建立重度楔状缺损牙模型,各组按分组内容进行牙体修复,用电子万能试验机进行加载,测试各组的抗折载荷,并记录样本所承受的最大负荷。结果抗折载荷的比较C1-1组>C2-1组、C1-2组>C2-2组、C1-3组>C2-3组,上述差异均具有显著性(P<0.05);C1-3组抗折载荷最高(P<0.01),B组最低(P<0.01),A、C1-1组、C2-2组三组间抗折载荷比较差异无显著性意义(P>0.05)。结论重度楔状缺损患牙修复时保留颊侧悬突牙体对承受抗折载荷有利,纤维桩能增强重度楔状缺损牙的抗折能力。Objective This study aimed to investigate the effects of different tooth preparations on the lracture strength with severe wedgeshaped defect restored by different methods. Method A total of 64 human mandibular premolars were randomly divided into three groups, Group A: 8 untreated premolars; Group B: untreated severe wedge-shaped defects premolars; Group C: severe wedge-shaped defects premolars were restored. According to whether the teeth above the wedge-shaped defect was removed (represented by C2) or not (represented by C1), Then each group was divided into 3 subgroups with one restored with fiber posts and a light-curing composite resin (represented by 1), one restored with fiber posts and resin cores and cast full crowns(represented by 2), and one restored with cast post-and-core and full crowns (represented by 3) after teeth preparations. A total of 64 human mandibular premolar teeth were randomly divided into 8 groups: A,B, Cl-1, C1-2, C1-3, C2-1, C2-2, C2-3,each group 8 teeth. All the teeth were prepared and restored accordingly and then mounted on an electronic pressure universal testing machine. The maximum fracttlre strength was recorded. Result The fracture strength of Group C 1-1 〉 that of Group C2-1, Group C1-2〉Group C2-2, and Group C1-3〉Group C2-3 with significant differences(P〈0.05). Group C1-3 had the highest fracture strength (P〈0.01), while Group B had the lowest fracture strength (P〈0.01). There were no significant difference among the fracture strength of Group A, Group CI-1 and Group C2-2(P〉0.05). Conclusion Our findings indicated that the fi'acture resistance of the severe wedge-shaped defected premolar can be improved.by fiber reinforced composite post and dentin above wedge shaped defect remained.
