摘 要:目的观察颅内动脉瘤切除手术前短期使用抗菌药物预防院内感染的疗效。方法试验分为二期进行,2006年至2009年(A期)对颅内动脉瘤手术(ICAAS)术后患者术前未采用抗生素预防性用药,患者术后出现感染后开始行抗感染治疗;2009年至2013年(B期),当发现术后院内感染率偏高的问题后,采用短期使用抗菌药物预防ICAAS患者的院内感染。比较两期患者院内感染的发生率和抗生素使用量。结果通过预防性使用抗菌药物,ICAAS患者院内感染发病率由78.4%降至29.8%(P〈0.01)。抗菌药物使用剂量由100.4DDD/100床日降至85.4DDD/100床日。结论神经外科是多数医院感染的高发病区,为了有效控制院内感染,对于重大手术,在术前应预防性使用抗生素。Objective To observe the curative effect of antibacterial drugs on the prevention of nosocomial infection before intracranial aneurysm operation. Methods The experiment was divided into two phases, in 2006 - 2009 (A) on postoperative patients after intracranial artery aneurysm surgery (ICAAS) without using any antibiotic prophylaxis, when patients with postoperative infection, we start anti infection treatment, in the second period (B, 2009 -2013 ), we used the short-term antibiotics for the prevention nosocomial infection in ICAAS patients. The incidence of infection and antibiotic consumption of the two period were compared. Results After prophylactic use of antibiotics, the incidence nosocomial infection rate decreased from 78.4% to 29.8% (P 〈 0. 01 ). Antibiotic dose was reduced from 100. 4 DDD/100 bed days to 85.4 DDD/100 bed days. Conclusions Department of neurosurgery is the most nosocomial infectious ward, in order to effectively control the nosoeomial infection, in the major operation, antibiotics can be used in preoperation.
