摘 要:目的探讨纳米碳在甲状腺癌手术中的作用。方法随机将33例甲状腺微小乳头状癌患者分为试验组与对照组,试验组于术前1 d行超声引导下甲状腺内注射纳米碳混悬液。统计清扫淋巴结总的数量、淋巴结转移的数量及误切甲状旁腺的数量。结果试验组平均清扫淋巴结9.12±4.23枚,对照组平均8.75±2.25枚,两组差异有统计学意义(P=0.002)。试验组发现1例误切的甲状旁腺,对照组发现2例误切的甲状旁腺,两组差异有统计学意义(P=0.008)。试验组淋巴结转移率为23.22%,对照组为19.37%,两组差异无统计学意义(P=0.301)。结论纳米碳在甲状腺癌手术中淋巴结显像效果好,同时甲状旁腺不被染黑,清扫黑染淋巴结,可以彻底清扫淋巴结,降低甲状旁腺被误切的发生率。Objective To explore the effect of carbon nanoparticle suspension injection in thyroid carcinoma. Methods Forty-three patients with papillary thyroid mierocarcinoma were randomly divided into two groups:the trial group and control group. In the trial group,carbon nanoparticle was ultrasound-guided injected into thyroid gland 1 day before the operation. The numberof lymph nodes,metastated lymph nodes and parathyroid glands in specimen were calculated. Results The average number of lymph nodes was (9.12±4.23) and (8.75±2.25) in the trial group and control group respectively,the difference was statistically significant (P=0.002). Parathyroid gland tissue was found in 1 specimens of the trial group and 2 specimens of the control group respectively,the difference was statistically significant (P=0.008). The lymph node metastasis rates were 23.22% and 19.37% in the trial group and control group respectively,and no statistical difference was observed(P=0.301). Conclusions Compartment lymph node can be well stained black by using carbon nanoparticle injection in thyroid carcinoma. Parathyroid gland can be preserved by dissecting the black tissue and retaining the non-black tissue,and compartment lymph node can be radically dissected at the same time.
