同时性原发性子宫内膜和卵巢双癌的临床分析

摘　　要：目的分析总结原发性子宫内膜和卵巢双癌的临床症状、病理特征、诊治及预后。方法回顾性分析同时发生子宫内膜癌和卵巢癌的8例患者的临床病理资料以及诊治、随访情况。结果异常阴道流血或排液为患者就诊主要原因(6/8),2例因盆腔包块而就诊。所有患者均行全面分期手术,术后病理检查提示8例均为子宫内膜样腺癌,5例为卵巢子宫内膜样腺癌,2例卵巢浆液性腺癌,1例卵巢透明细胞癌。所有患者术后化疗,部分患者补充放疗,随访16~48个月,平均随访时间为29个月,1例复发治疗中。结论子宫内膜和卵巢原发性双癌术前诊断困难,如术中病理分期较早,根治性手术联合化疗为主要治疗方式。Objective To summarize the clinicol symptoms,pathological features,diagnosis,therapy and prognosis of primary carcinoma simultaneously involving uterine corpus and ovary. Methods The clinicopathological and follow-up data of 8 patients with synchronous primary cancer of endometrium and ovary were retrospectively reviewed. Results The main complain were abnormal vaginal bleeding or fluid （6/8）,meanwhile 2 patient with pelvic mass. All the 8 patient received comprehensive staging laparotomy,were confirmed endometrioid adenocar- cinoma,5 patients with ovary endometrioid adenoearcinoma,2 patients with serous adenocarcinoma,lpatient with clear cell adenocarcinoma. All the 8 patients had postoperative chemothera,2 patients received radiation therapy. they were all followed up for 16 to 48 months and all survived except 1 patient still receiving chemocal therapy because of recurrence. Conclusion Correct preoperative diagnosis of primary carcinoma simultaneously involving uterine corpus and ovary is difficultly made before surgery,however when pathological staging is early more,the main treatment still is radical surgery supplemented with postoperative chemotherapy.
