摘 要:目的:应用锥形束CT(CBCT)评价老年人下颌磨牙热牙胶根管充填质量,与口内数字片做对照,更深入探讨CBCT在老年人牙齿根管治疗中的作用。方法:选择2015年6月~2016年8月在大连市口腔医院门诊就医,因下颌磨牙(第三磨牙除外)需根管治疗的老年人51例80颗牙齿253个根管。术前拍摄CBCT,用热牙胶垂直加压法根管充填,术后同时采用口内数字片和CBCT两种不同影像评价根管充填质量,并用统计学方法比较二者间的差异。结果:充填253个根管,以根管为单位评价根管充填质量。从根管充填长度评价口内数字片有9.1%的误差率;经χ2检验,口内数字片和CBCT评价结果有显著性差异(P<0.05)。从根充密合度评价口内数字片有2.0%的误差率,经χ2检验,口内数字片和CBCT评价结果无显著性差异。结论:CBCT较口内数字片评价老年人下颌磨牙热牙胶根充质量具有很高的准确性,CBCT在老年人磨牙根管治疗过程中具有临床指导优势。Objective:To evaluate the quality of hot gutta-percha root canal filling in the mandibular molar in the elderly by cone-beam CT.Methods:Eighty molars with 253 root canals in 51 geriatric patients from June 2015 to August 2016 in the Stomatology Hospital of Dalian were selected.Before the operation,the molars were scanned with cone-beam CT.The root canals were filled with hot gutta-percha and vertical pressurized.The quality of root canal filling were evaluated by cone-beam CT and digital images.Results:The error rate of root canal length observed by digital images was 9.1%.The difference of evaluation result of digital images in mouth and conebeam CT was statistical significance(P〈0.05).The error rate of the root canal adaptation of digital images at 2.0% was evaluated.The differences of digital images and cone-beam CT results were no statistical significance.Conclusion:Cone-beam CT has a higher accuracy in evaluating the hot gutta-percha root-filling quality of mandibular molar of the geriatric patients than digital images.
