小切口白内障手术联合隧道内小梁切除术治疗白内障青光眼临床效果分析

摘　　要：目的观察在白内障青光眼患者治疗过程中应用小切口白内障手术联合隧道内小梁切除术治疗的临床效果。方法 60例(66眼)白内障青光眼患者,均给予小切口白内障手术联合隧道内小梁切除术治疗,观察临床治疗效果。结果随访6个月,术后视力<0.05的眼数8眼(12.12%),术后视力在0.05~0.3的眼数10眼(15.15%),术后视力在0.3~0.5的眼数38眼(57.58%),术后视力>0.05的眼数10眼(15.15%)。其中,术后视力存在不同程度提高的患眼为58眼(87.88%),视力不提高8眼(12.12%)。66眼术前眼压为(36.38±6.66)mm Hg(1 mm Hg=0.133 kPa),术后1周眼压为(17.59±6.10)mm Hg,术后1个月眼压为(16.92±4.55)mm Hg,术后3个月眼压为(15.98±3.85)mm Hg,术后6个月眼压为(16.22±3.88)mm Hg,患者术后各个观察时间点的眼压均低于术前,差异均具有统计学意义(t=16.9023、19.6004、21.5438、21.2487,P<0.05)。结论在白内障青光眼患者治疗过程中应用小切口白内障手术联合隧道内小梁切除术治疗的临床疗效较为显著,可在临床推广应用。Objective To observe the clinical effect of small incision cataract surgery combined with tunnel trabeculectomy in the treatment of cataract and glaucoma. Methods A total of 60 cases （66 eyes） of cataract and glaucoma patients all received small incision cataract surgery combined with tunnel trabeculectomy,and their clinical treatment effect was observed. Results After 6 months of follow-up, there were 8 eyes （12.12%） with postoperative eyesight 〈0.05, 10 eyes （15.15%） with postoperative eyesight in 0.05-0.3, 38 eyes （57.58%） with postoperative eyesight in 0.3-0.5, and 10 eyes （15.15%） with postoperative eyesight 〉0.05. There were 58 eyes （87.88%） with postoperative eyesight improvement in various degree, and 8 eyes （12.12%） without eyesight improvement. 66 eyes had preoperative eye pressure as （36.38 ±6.66） mm Hg （1 mm Hg=0.133 kPa）, postoperative 1 week as （17.59 ± 6.10） mm Hg, postoperative 1 month as （16.92 ± 4.55） mm Hg, postoperative 3 month as （15.98 ± 3.85） mm Hg, and postoperative 6 months as （16.22± 3.88） mm Hg. Patients had lower eye pressure in postoperative observation time points than preoperation, and the difference had statistical significance （t=16.9023, 19.6004, 21.5438, 21.2487, P〈0.05）. Conclusion Small incision cataract surgery combined with tunnel trabeculectomy provides significant clinical effect in the treatment of cataract and glaucoma. This method is worth clinical promotion and application.
