摘 要:目的观察胎盘多肽注射液对经肝动脉化疗栓塞术(TACE)治疗原发性肝癌患者肝功能和免疫功能的影响。方法回顾性分析2013年1月—2015年12月收治98例原发性肝癌患者的临床资料,按治疗方法分为对照组和观察组各49例。其中对照组采用TACE治疗,观察组在对照组基础上加用胎盘多肽注射液,连续治疗4个疗程后,进行疗效评价。比较两组患者治疗前后CD3+、CD4+、CD8+、CD4+/CD8+和自然杀伤(NK)细胞等免疫功能指标及测定血液中总胆红素(Tbil)、直接胆红素(Dbil)、间接胆红素(Ibil)、天冬氨酸转氨酶(AST)及丙氨酸转氨酶(ALT)等肝功能指标,同时并记录两组患者治疗期间的不良反应发生情况。结果观察组总有效率明显高于对照组(P<0.05)。治疗后观察组CD3+、CD4+、CD4+/CD8+、NK细胞高于治疗前和对照组(P<0.05);而CD8+细胞低于治疗前(P<0.05)。治疗后对照组CD3+细胞高于治疗前,CD4+细胞和CD4+/CD8+低于治疗前(P<0.05)。治疗后观察组Tbil、Dbil、Ibil、AST及ALT水平均低于治疗前和对照组(P<0.05)。且治疗期间,观察组各种不良反应发生率均明显低于对照组(P<0.05)。结论胎盘多肽注射液联合TACE方案治疗原发性肝癌能提高患者免疫功能和改善肝功能,同时降低化疗药物的不良反应。Objective To observe effects of Placenta Polypeptide Injection on liver function and immune func- tion of patients with primary liver cancer after transcatheter arterial chemoembolization (TACE) treatment. Methods Clinical data of 98 patients with primary liver cancer admitted during January 2013 and December 2015 was retrospective- ly analyzed. The patients were divided into control group and observation group (n = 49 for each group). Control group was treated with TACE, while observation group was added with Placenta Polypeptide Injection on the basis of treatment for control group. After 4 courses of treatment, the curative effect was evaluated. Levels of immune function indexes such as CD3 + , CD4 + , CD8 + , CD4 +/CD8 + and natural killer (NK) cells before and after treatment were compared be- tween the two groups. Liver function indexes such as total bilirubin (Tbil) , direct bilirubin (Dbil) , indirect bilirubin (Ibil) , aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels in blood were determined, and in- cidence rates of adverse reactions during treatment were recorded simultaneously in two groups. Results The total effec- tive rate in observation group was significantly higher than that in control group (P 〈 0.05 ). After treatment, in observa- tion group, CD3 + , CD4 + , CD4 +/CD8 + and NK cells levels were higher than those before treatment and in control group (P 〈0.05) ; CD8 + cells level was lower than that before treatment (P 〈 0. 05). After treatment, in control group, CD3 + cells level was higher, while CD4 + cells and CD4 +/CD8 + levels were lower than those before treatment (P 〈 0. 05).
