肛门直肠压力测定在直肠前突PPH手术中的应用分析

摘　　要：目的探讨分析肛门直肠压力测定在直肠前突手术中的临床应用效果。方法选取直肠前突患者32例,均接受吻合器痔上黏膜环切术(PPH)治疗,手术前后均行肛门直肠压力测定。比较手术前后检测指标差异,评估手术治疗效果。结果 32例患者术后直肠指检及排便造影示前突消失,排便困难症状缓解。随访12个月,有效率为81.25%(26/32)。患者术后肛管静息压、肛管最大缩窄压、括约肌功能长度、直肠初始感觉阈值量、初始排便阈值和最大耐受阈值均明显下降,差异均有统计学意义(P<0.05)。结论直肠前突患者肛管直肠测压相关指标均明显高于正常水平,通过肛管直肠测压可对直肠前突PPH手术效果进行评估,是判定直肠前突治疗疗效的指标之一。Objective To evaluate the clinical effect of anorectal manometry in procedure for prolapse and hemorrhoids（ PPH） operation in treatment of rectocele. Methods 32 cases of rectocele treated by PPH were chosen. Anorectal manometry were tested before/after operation. The parameters differences were compared for assessing the effect of PPH operation. Results Rectocele disappears and the defecation difficulty relieves after PPH operation using rectal examination and defecography. The effective rate is higher at 81. 25%（ 26/32） after a follow-up examination of 12 months. There were significantly decreased in anal canal resting pressure,anal canal maximum constrictive pressure,sphincter function length,initial sensory threshold volume,initial defecation threshold and maximum tolerable threshold after PPH operation（ P 〈0. 05）. Conclusion The relative parameters of anorectal manometry are higher than normal level.Anorectal manometry is used to assess the PPH operation,which is a promising parameter for evaluating the clinical effect of rectocele.
