摘 要:目的探究SPECT/CT骨断层融合显像在恶性肿瘤骨转移诊断中的增益价值。方法选取158例经病理确诊的恶性肿瘤患者,均行全身骨平面显像后对可疑骨转移或不明原因骨痛部位进行SPECT/CT骨断层融合显像,然后对全身骨显像与SPECT/CT骨断层融合显像诊断结果进行比较分析。结果 158例恶性肿瘤患者最终确诊骨转移者67例,良性病变者91例。SPECT/CT骨断层融合显像诊断骨转移瘤的灵敏度(95.5%)与全身骨显像(86.6%)相比较差异无统计学意义(χ~2=3.295,P=0.069),而特异性及准确性明显高于全身骨显像(χ~2=4.031,P=0.045;χ~2=7.229,P=0.007)。结论全身骨显像仍作为筛查骨转移瘤的首选检查方式,SPECT/CT骨断层融合显像能够同时提供代谢与解剖信息,大大提高骨转移诊断的准确性,在诊断骨转移瘤方面具有较高的增益价值。Objective To explore the diagnostic value of SPECT/CT bone fusion imaging in detection of malignant bone me- tastases. Methods Totally, 158 patients suspected for bone metasis and with bone pain of unknown origin were included in this study. All cases underwent SPECT/CT bone fusion imaging after radionuelide bone scan (BS). The results of the above-men- tioned examinations were compared and analyzed with chi-square test. Results 67 of 158 patients with malignant tumors were e- ventually diagnosed with bone metastases. The diagnostic sensitivity of BS and SPECT/CT was 86.6% and 95.5% ( x^2 = 3. 295, P =0.069), while, the specificity was 82.4% and 92.3%(x^2 =4.031, P =0.045), and the accuracy was 84.2% and 93.7% ( x^2 = 7. 229, P = 0. 007 ) , respectively. Conclusion Radionuelide bone scan is still the first choice for the screening Of bone metastases. Owing to combine the metabolism imaging with the anatomical imaging, SPECT/CT bone fusion imaging can ef- fectively increase the diagnostic accuracy of metastatic bone lesions in tumor patients.
