摘  要:目的通过问卷调查评估手机应用软件(APP)系统支持临床路径沟通工作的可行性和效果。方法某院小儿外科自研一种医用APP系统,2016年2月至同年7月被应用于科室一、二病区开展的路径信息化沟通与传统沟通的非随机对照实验,期间两病区分别有427例和331例路径患方接受了有关沟通效果的问卷调查,分别收回406份(实验组)和312份(对照组)有效问卷。结果实验组和对照组的调查结果如下:知晓自己接受路径管理(81.7%vs.49.0%),有效阅读过路径告知书(23.9%vs.10.9%),有效阅读过路径宣教知识(14.0%vs.4.8%),认为自己对路径有基本理解(42.1%vs.8.3%),认可路径对自己有实际价值(36.2%vs.8.6%),实际关注过自己路径执行(32.5%vs.6.7%),能够体验到路径的运行(46.6%vs.4.8%),知晓自己路径的最终结果(28.3%vs.3.2%),能对路径管理形成评价(35.7%vs.9.3%),对路径沟通工作认可(78.3%vs.18.3%)。实验组的肯定回答率均高于第二病区,差异均具有统计学意义。但系统用户关注的信息重点是路径化的住院指导而不是路径本身,部分患方对系统存在较多顾虑和困难。结论目前患方已经基本能够接受和配合APP医患沟通系统。信息化的沟通方式可有效促进患方对临床路径的关注和理解。Objectives To evaluate the feasibility and communication effect of an APP system on the clinical pathways by questionnaire survey. Methods From February to July in 2016, 427 parents whose children underwent CP in the first ward of our department where the APP system was running and 331 ones in the second ward without the support of information platform participated in an investigation. Ultimately, 406 and 312 effective questionnaires from the first and second ward were received respectively. The outcomes in the two wards were compared by using Z2 test, a P-value less than 0. 05 as statistically significant. Results Respectively 81.7% and 49.0% of the respondents in the first and second ward knew their own children had received a management of CP, 23.9% and 10. 9% had read the CP Notice, 14. 0% and 4. 8% had read introductory articles about CP, 42. 1% and 8. 3% thought they understood CP, 36.2% and 8.6% approved CP, 32. 5% and 6. 7% had kept the focus on their own CP, 46. 6 and 4. 8% had discovered the operation of the CP, 28.3% and 3.2% had received the result of their own CP, 35.7% and 9. 3% had gotten clever views on CP, 78.3% and 18.3% approved the communication on CP. The effect of communication about CP in the first ward with the help of the APP system was better than in the second ward. There were statistically significant differences in the two groups. Conclusions Most patients in China would have already accepted doctor-patient communication by APP system. Information communication could promote the attention and understanding to CP among patients or family members effectivelv.
