眼轮匝肌部分切除联合A型肉毒毒素治疗眼轮匝肌肥厚型睑袋的临床研究

摘　　要：目的：探讨应用眼轮匝肌部分切除联合A型肉毒毒素治疗眼轮匝肌肥厚型睑袋的临床效果。方法：选择我院在2014年6月-201 6年6月期间收治的72例眼轮匝肌肥厚型睑袋患者为研究对象,并随机分为注射组、切除组及联合组,每组各24例;注射组采用A型肉毒毒素注射治疗,切除组行眼轮匝肌部分切除术,联合组应用眼轮匝肌部分切除术联合A型肉毒毒素,治疗后随访12周。比较三组患者治疗效果、视觉模拟评分法（VAS）行满意度评分、不良反应发生率。结果：联合组治疗效果显著优于注射组和切除组,差异有统计学意义（P〈0.05）;采用视觉模拟评分法（VAS）进行满意度评分,联合组显著优于注射组和切除组,差异有统计学意义（P〈0.05）;不良反应发生率与注射组和切除组差异无统计学意义（P〉0.05）。结论：眼轮匝肌部分切除联合A型肉毒毒素治疗眼轮匝肌肥厚型睑袋可有效改善患者临床症状,提高患者满意率,而且可保证安全性,对提升患者生活质量有重要意义。Objective To investigate the clinical effect of the treatment of orbicularis oculi muscle hypertrophy with partial excision of the orbicularis oculi muscle combined with Botulinum Toxin Type A. Methods 72 cases of orbicularis muscle hypertrophy patients in our hospital during June 2014 - June 2016 as the research object, and they were randomly divided into injection group, resection group and combined group, 24 cases in each group. Injection group was treated by Botulinum Toxin Type A injection, resection group was treated with partial resection of the orbicularis oculi muscle, the combined group was treated with orbicularis muscle resection combined with Botulinum Toxin Type A injection, follow-up the effect after 12 weeks. Then, we compared the therapeutic effects of the three groups, graded the score by the visual analogue scale （VAS）, and compared the incidence of adverse reactions between the groups. Results The therapeutic effect of the combined group was significantly better than the injection group＇s and the resection group＇s, the difference was statistically significant （P〈0.05）. The visual analogue scores（VAS） after operation for 8 weeks of the combined group were significantly better than the injection group＇s and the resection group, the difference was statistically significant （P〈0.05）.The incidence rate of adverse reaction of injection group and the resection group had no significant difference （P〉0.05）. Conclusion The orbicularis muscle resection combined withA in the treatment of orbicularis muscle hypertrophy can effectively improve the clinical symptoms of Botulinum Toxin Type A, improve patient＇s satisfaction rate, and ensure safety.It has important significance to improve the quality of life.
