右美托咪定在义眼台置换术中的临床应用效果

摘　　要：目的评价在右美托咪定镇静状态下进行局部麻醉义眼台置换术临床效果及安全性。 方法32例术前15 min静脉滴注右美托嘧定（1.0 μg/kg），10～15 min滴注完毕，达到Ramsay镇静VI级后，再用0.75%罗哌卡因与2%利多卡因等量混合，分别行患眼球后及结膜下麻醉，检查患者反应恢复到Ramsay镇静Ⅲ级后开始手术操作并维持右美托咪定静脉滴注。 结果全组32例术中均无疼痛感并能很好配合手术，术后29例（90.63%）疼痛、结膜水肿及恶心呕吐症状均轻微；3例（9.37%）疼痛较明显，对症治疗后症状缓解。 结论在局部麻醉义眼台置换术中应用右美托咪定可减轻术中疼痛及术后反应。Objective To evaluate the efficacy and safety of Dexmedetomidine applied in orbital implant replacement surgery in the downlink sedation. Methods Thirty-two patients were intravenoused infusion of Dexmedetomidine pyrimidine 15 rain before surgery （ 1. 0 μg/kg）. After 10-15 min the drug dripped out,all cases reached Ramsay sedation grade VI. Local anesthesia with 0.75% rupivacaine mixed with 2% lidocaine equivalently was respectively treated with retrobulbar and subconjunctival anesthesia. After the patients recovered to Ramsay grade HI reaction the surgery started. Results Thirty-two patients could cooperate with the operation with no pain. There were 29 patients （ 90.63% ） had very mild pain, conjunetival edema, nausea and vomiting after operation. The severe pain was in the 3 cases（9.37% ）, the pain was significantly relieved after symptomatic treatment. Conclusion The application of Dexmedetomidine in orbital implant replacement surgery can reduce postoperative pain and posto-perative reaction.
