双歧三联活菌胶囊对白血病化疗相关性腹泻患者肠黏膜屏障的保护作用

摘　　要：目的探讨双歧三联活菌胶囊对白血病化疗相关性腹泻(CID)患者肠黏膜屏障的保护作用。方法选取白血病化疗发生CID患者92例,随机分为观察组和对照组各46例。两组患者均予口服蒙脱石散、静脉或口服补液和维持水电解质酸碱平衡等基础治疗。观察组患者予双歧三联活菌胶蔻420 mg/次,3次/d,温水送服。检测两组患者治疗前和治疗72 h后血清内毒素(ET)、二胺氧化酶(DAO)、D-乳酸和肿瘤坏死因子-α(TNF-α)水平变化,并比较临床疗效。结果治疗72h后,两组血清ET、DAO、D-乳酸水平及TNF-α均有不同程度下降(P<0.05或P<0.01)、且在下降幅度方面观察组大于对照组(P<0.05);在临床总有效率上,观察组高于对照组(χ~4=4.39,P<0.05)。结论双歧三联活菌胶囊治疗白血病CID的疗效确切,能明显降低血清ET、DAO和D-乳酸及TNF-α水平,保护与修复肠黏膜屏障功能。Objective To discuss the protection effect of bifid triple viable capsules combined on intestinal mucosal barrier of patients with chemotherapy-induced diarrhea（CID） with leukemia. Methods 92 cases of patients with leukemia were divided into observation group and control group, with 46 cases in either group. The patients in two groups were given oral smectite powder, intravenous or oral infusion, maintenance of water and electrolyte acid-base balance and other basic medical treatment, and the patients in observation group were given 420 mg bifid triple viable capsules pre time, three times a day with warm boiled water. The changes of serum endotoxin（ET）, diamine oxidase（DAO）, D-Lactic acid and TNF-α of patients in two groups before and 72 hours after treatment were detected, and the clinical curative effect were compared as well. Results After 72 hours treatment, serum ET, DAO, D-Lactic acid and TNF-α levels in two groups were obviously declined than before（P〈0.05 or P〈0.01）, and the declining rate of patients in traditional and western medicine group were much higher than those in western medicine group after medical treatment（P〈0.05）. The total clinical efficiency of patients in traditional Chinese and western medicine group was much higher than that in western medicine group（x2=4.39, P〈0.05）. Conclusion Bifid triple viable capsules has more favorable curative effect on CID with Leukemia, which can obviously reduce serum ET, DAO, D-Lactic acid and TNF-α levels, protect and remedy intestinal mucosal barrier function.
