中间入路腹腔镜辅助右半结肠癌根治术的临床研究

摘　　要：目的比较分析腹腔镜辅助右半结肠癌根治术经中间入路以及侧方入路的疗效,旨在为结肠癌的治疗探寻一种安全和疗效最佳的手术方案。方法纳入的52例患者为2015年6月~2016年6月我院收治的行腹腔镜辅助右半结肠癌根治术患者,其中根据手术入路不同进行分组,26例采用中间入路设立为中间入路组,26例采用侧方入路设立为侧方入路组。比较两组患者的手术时间、术中出血量、术后住院时间,以及比较分析两组在清扫淋巴结总数、清扫淋巴结阳性数方面的差异。结果中间入路组与侧方入路组在手术时间、术中出血量、术后住院时间方面比较,差异无统计学意义(P>0.05),中间入路组与侧方入路组在清扫淋巴结总数、清扫淋巴结阳性数方面比较,差异有统计学意义(P<0.05),其中中间入路组清扫淋巴结总数、清扫淋巴结阳性数明显多于侧方入路组,淋巴结清扫更彻底。结论腹腔镜辅助右半结肠癌根治术经中间入路以及侧方入路均具有较好的手术疗效,两种不同入路在手术时间、术中出血量、术后住院时间方面方面比较未见明显差异,但中间入路在淋巴结清扫方面具有显著优势,对淋巴结清扫更彻底。Objective To compare and analyze the curative effect of laparoscopy-assisted radical resection of the right hemicolon carcinoma by medial approach and lateral approach, aiming at exploring a safe and most effective surgical procedure for the treatment of colon cancer. Methods 52 patients were admitted to our hospital from June 2015 to June 2016, who were given laparoscopy-assisted radical resection of the right hemicolon carcinoma. They were assigned to groups according to different surgical approaches. 26 patients who were given medial approach were assigned to the medial approach group, and 26 patients who were given lateral approach were assigned to the lateral approach group. The operation time, volume of bleeding during surgery and postoperative length of stay were compared between the two groups. And the difference in the total number of lymph node dissection and the number of positive lymph node dis- section were compared and analyzed between the two groups. Results There was no statistically significant difference in the operation time, volume of bleeding during surgery and postoperative length of stay between the medial approach group and the lateral approach group（P〉0.05）. There was statistically significant difference between the medial approach group and the lateral approach group in the total number of lymph node dissection and the number of positive lymph node dissection（P〈0.05）. The total number of lymph node dissection and the number of positive lymph node dissection in the medial approach group were significantly higher than that in the lateral approach group, and the dissection of lymph nodes in the medial approach group was more thorough. Conclusion Laparoscopy-assisted radical resection of the right hemicolon carcinoma has a good therapeutic effect through the medial approach and the lateral approach.
