仲景水肿治法浅析

摘　　要：仲景治疗水肿病有三大法则——发汗、利小便、攻下逐水,发汗法主要适用于"腰以上肿"、水肿病兼表证者;利小便法是治疗水肿的基本法则,主要用于"腰以下肿";攻下逐水主要用于水肿实证、重症,是急则治其标的治法。仲景发汗、利小便、攻下逐水的三大治则是在《黄帝内经》"开鬼门""洁净府""去菀陈莝"的理论基础上演变发挥而来,他在《黄帝内经》理论基础上又创造了温阳、益气、滋阴、活血等水肿虚证的治疗方法。仲景所创治疗水肿的理、法、方、药,从一千年前沿用至今,指导着历代医家临床用药,是现代临床治疗心源性、肾源性、肝源性、肺源性等各型水肿的主要治疗法则。仲景《伤寒论》和《金匮要略》中治疗水肿的理论及方剂丰富和发展了中医学理论宝库,值得继承和发扬。Zhang Zhongjing treated edema with three methods： sweating,promoting diuresis,purging water. Sweating is mainly for edema above waist and edema complicated with exterior syndrome; promoting diuresis is the basic rule for the treatment of edema,mainly used for edema below the waist; purging water is mainly used for severe edema of excess syndrome,and is also a method for treating the superficial manifestation in urgent cases. Zhongjing’s three major therapeutic principles of sweating,promoting diuresis and purging water were originated from ＂ opening the pores＂, ＂ cleansing the bladder＂ and ＂ removing the stagnation and wastes＂ in the Huangdi’ s Internal Classics. Based on the theories in Huangdi’ s Internal Classics,Zhang Zhongjing created therapeutic methods for edema of deficiency syndrome,including warming yang,benefiting qi,nourishing yin and activating blood. The theory,strategy,formula and drug created by Zhang Zhongjing were applied for over one thousand years,guiding the clinical drug application for doctors of generations,which is the main therapeutic principle for modern clinical treatment of cardiogenic,kidney-originated,liver-originated and lung-originated edema. Zhongjing’s theories and prescriptions for treating edema in Treatise on Cold Damage and Synopsis of the Golden Chamber have enriched the treasuryof traditional Chinese medicine theory,which is worth inheriting and developing.
