射频消融术在局部进展期胰腺癌中的临床应用

摘　　要：[目的]探讨射频消融术治疗局部进展期胰腺癌的临床疗效。[方法]本院收治的123例局部进展期胰腺癌患者，依据其姑息性手术中是否行射频消融术分为射频组和对照组，比较两组患者治疗及预后情况。[结果]123例患者均顺利完成手术，无围手术期死亡患者。射频组患者术后并发症发生率为21．7％（10／46），胰瘘发生率为6．5％（3／46），与对照组10．38％（8／77），1．3％（1／77）相比较差异均无显著性（P〉0．05）。射频组患者的半年、1年、2年生存率分为82．6％（38／46）、43．5％（20／46）、8．7％（4／46），平均生存时间为（13．5±0．8）个月；明显高于对照组55．8％（43／77）、27．27％（21／77）、0％和（9．1±0．3）个月，且差异均有显著性（P〈0．05）。[结论]射频消融术用于治疗局部进展期胰腺癌安全有效，其为胰腺癌患者的综合治疗提供了一种新的方法。[Objective]To analysis of radiofrequency ablation in the treatment of locally-advanced pancreatic carcinoma. [Methods]We retrospectively analyzed the clinical data of 123 patients with locally-advanced pancreatic carcinoma. All the patients were divided into a radiofrequency group and a control group according to the radiofre quency ablation surgery. And then the perioperative and prognosis data of the two groups were analyzed and compared. [Results]In our research, all patients were successfully completed surgery and there were no cases of perio- perative death. The incidence of postoperative complications of radiofrequency group was 21.7% （control group rate 10.38 % ） and the incidence of pancreatic fistula was 6.5 % （control group rate 1.3%）. There were no statistical differences between the two groups. The 6 months, 1 year and 2 years survival rates of radiofrequency group and control group were 82.4%, 43.5%, 7.6% and 55.8%, 28%, 0%, the average survival time were 13.5±0.8 months and 9.1±0.3 months. The prognosis of radiofrequency group was significantly better than that of the control group （ P〈0.05）. [Conclusion]Radiofrequency ablation surgery is safe and effective in the treatment of local ly-advanced pancreatic carcinoma.
