摘 要:目的探讨microRNA(miR)-657在肝细胞癌(肝癌)中表达及临床意义。方法本前瞻性研究对象为2015年4月至2015年11月在高州市人民医院接受诊治的19例肝癌患者,并选取同期手术的19例非肝癌患者作为对照。所有患者均签署知情同意书,符合医学伦理学规定。其中肝癌组中,男15例,女4例;年龄30~68岁,中位年龄48岁。对照组中,男7例,女11例;年龄23~77岁,中位年龄51岁。分别收集肝癌组织、癌旁组织和正常肝组织标本。采用RT-PCR检测miR-657表达水平。组织中miR-657表达量的比较采用单因素方差分析和LSD-t检验,miR-657表达与肝癌患者临床病理参数的关系分析采用t检验。结果肝癌组织中miR-657表达量为正常肝组织的(3.5±1.6)倍,癌旁组织为(1.6±0.6)倍。肝癌组织中miR-657表达量明显高于癌旁组织和正常肝组织(LSD-t=6.904,6.769;P<0.05),癌旁组织中miR-657表达量明显高于正常肝组织(LSD-t=4.277,P<0.05)。血清AFP>400μg/L、肿瘤直径>5 cm、合并门静脉癌栓、病理分级为Ⅲ~Ⅳ级、HBV-DNA定量≥500 copies/ml患者的肝癌组织中miR-657表达量明显升高(t=-2.249,-2.925,-2.424,-3.143,-2.509;P<0.05)。结论肝癌组织中miR-657表达可能与肝癌的发生、转移、侵袭有关,其可作为肝癌复发预测指标。Objective To investigate the expression and clinical significance of microRNA (miR)-657 in hepatocellular carcinoma (HCC). Methods Nineteen patients with HCC who were diagnosed and treated in Gaozhou People's Hospital between April 2015 and November 2015 were enrolled in this prospective study. And 19 non-HCC patients who received surgery at the same period were chosen as the control. The informed consents of all patients were obtained and the local ethical committee approval was received. In the HCC group, there were 15 males and 4 females with the age ranging from 30 to 68 years old and a median age of 48 years old. In the control group, there were 7 males and 11 females with the age ranging from 23 to 77 years old and a median age of 51 years old. HCC tissues, paracarcinoma tissues and normal liver tissues were respectively collected. The expression level of miR-657 was detected by RT-PCR. The expression levels of miR-657 in the tissues were compared by one-way analysis of variance and LSD-t test. The relationship between the expression of miR-657 and the clinicopathological characteristics of HCC patients was analyzed using t test. Results The expression of miR-657 in the HCC tissues andthe paracarcinoma tissues was respectively (3.5±1.6) and (1 .6±0.6) times of that in the normal liver tissues. The expression of miR-657 in the HCC tissues was significantly higher than that in the paracarcinoma tissues and normal liver tissues (LSD-t=6.904, 6.769; P〈0.05). The expression of miR-657 in the paracarcinoma tissues was significantly higher than that in the normal liver tissues (LSD-t=4.277, P〈0.05). The expression of miR-657 was significantly up-regulated in the HCC tissues of the patients with serum AFP〉 400μg/L, tttmor diameter〉5 cm, complicated with portal vein tumor thrombus, pathological grade Ⅲto Ⅳ and HBV-DNA titer≥500 copies/ml (t=-2.249, -2.925, -2.424, -3.143, -2,509; P〈0.05).
