23G玻璃体切割手术在白内障超声乳化术中处理后囊膜破裂的疗效观察

摘　　要：目的:观察23G玻璃体切割手术在白内障超声乳化术中处理后囊膜破裂的临床疗效。方法:本课题研究选取2014年12月-2016年4月本院眼科收治的60例白内障超声乳化术处理后囊膜破裂患者作为研究对象,60例患者共60只患眼,术中发生后囊膜破裂后,对患者实施23G玻璃体切割手术治疗,术后随访本组患者3个月,观察本组患者的视力改善情况。结果:本组60例患者均顺利完成手术,术后6、12、24 h的VAS评分(疼痛视觉模拟评分)分别为(5.2±1.1)分、(3.6±0.9)分和(2.5±0.8)分,不同时间段比较差异均有统计学意义(P<0.05)。术后共2例患者发生并发症,均为瞳孔轻度上移,并发症发生率为3.3%。术后评估60例患者的视力,结果显示共52例患者视力改善,所占比例为86.7%。结论:采用23G玻璃体切割手术处理白内障超声乳化术中后囊膜破裂,疼痛感轻,并发症少,安全性高,能够有效改善患者视力,临床疗效确切。Objective： To observe clinical curative effect of 23G vitreous incision on posterior capsule rupture in phacoemulsification of cataract.Method： 60 cases with posterior capsule rupture in phacoemulsification of cataract admitted in our hospital were selected from December 2014 to April 2016, 60 cases with 60 affected eyes.After posterior capsule ruptured in operation, 23G vitreous incision was used for treatment.Three months＇ follow-up visits to patients were adopted after treatment to observe the improvement of visual acuity.Result： The operations on 60 cases were performed carefully, postoperative VAS score in 6, 12 and 24 h （visual analogue scale ） were respectively （ 5.2±1.1 ） score, （ 3.6±0.9 ） score and （ 2.5±0.8 ） score, compared them, the differences were statistically significant （P〈0.05） .After operation, complication happened on two cases, both being mild upward move of pupils, the complication rate was 3.3%.After operation, the eyesight of 52 cases improved, accounting for 86.7%.Conclusion： The adoption of 23G vitreous incision to treat the posterior capsule rupture happening at phacoemulsification of cataract can relieve pain, reduce complication, and significantly improve the eyesight of patients, plus high safety and clinical efficacy.
