40例帕金森病患者非运动症状的时序性分析

摘　　要：目的通过对帕金森病患者各种非运动症状发生前后的时序进行分析,探讨帕金森病患者非运动症状出现早晚的临床特征。方法对40例帕金森病患者进行"帕金森病常见的非运动症状问卷"结合部分常见的运动症状进行调查,并结合性别、病程等可能的影响因素对非运动症状发生前后时序进行排列评估。结果在40例帕金森病患者中,味觉或嗅觉减退或丧失是最先开始出现的症状,在确诊帕金森病前就开始出现(平均发病前1.2个月出现),且发生率高[57.5%(23/40)];其次出现时间较早的症状依次是皮炎、感到难以入睡或失眠、因为头晕或失去知觉而摔倒,这3个症状平均在确诊后半年内出现,发生率相对较低[分别为12.5%(5/40),27.5%(11/40),5.0%(2/40)];较晚出现的症状有幻觉(看到或听到一些你知道或者被告知并不存在的事情)、发声困难或异常、有不明原因的疼痛(并非已知原因导致如关节炎)、流涎、色觉异常,这5个症状平均在确诊发病6年以后才出现。结论帕金森病患者的非运动症状中,首先出现的是味觉或嗅觉的减退,且早于运动症状;其次是皮炎,睡眠障碍,头晕,较晚出现的是幻觉、发声困难或异常、不明原因的关节疼痛、流涎、色觉异常。Objective To investigate the clinical characteristics of non-motor symptoms onset in patients with Parkin- son＇s disease by analyzing the timing of various non-motor synlptoms in patients with Parkinson＇s disease. Methods 40 patients with Parkinson＇s disease were investigated ＂Parkinson＇s disease common non-motor symptom questionnaire＂ combined with some common symptoms of motor.And the timing of non-motor symptoms was assessed with the combi- nation of sex, duration and other possible factors. Results In 40 patients with Parkinson＇s disease, taste or smell dimin- ishment or loss were the first symptoms to appear, which happened before the diagnosis of Parkinson＇s disease （1.2 months before the onset on average） and had a high incidence [57.5% （23/40）]. The following symptoms were dermatitis, feeling hard to fall asleep or insomnia,falling down because of dizziness or loss of consciousness, and the three symp- toms occurs in the six months after the initial diagnosis on average with relatively low incidence [12.5% （5/40）, 27.5% （11/40）, 5.0%（2/40）]. Symptoms appearing later were illusions to see or hear something you know or are told that does not exist,dysphonia or abnormality, and unexplained pain which is not known to cause arthritis, salivation and abnormal color vision. The above symptoms were seen only after 6 years of diagnosis. Conclusion Among the non-motor symp- toms in Parkinson＇s disease patients, the first appearance is the taste or smell of decline, which is earlier than the movement symptoms, followed by dermatitis,sleep disorders,dizziness. And hallucinations, dysphonia or abnormalities, unexplained joint pain, salivation and ahnormal color vision appear late.
