成人共同性外斜视术后双眼远立体视功能重建及影响因素分析

摘　　要：目的:探讨成人共同性外斜视术后双眼远立体视功能重建情况及影响因素分析。方法:回顾性分析在我院进行共同性外斜视手术的成人患者122例的临床资料,根据发病年龄、手术年龄、斜视度将患者分组。患者术前均无同时视,术后6周用同视机检查是否出现双眼立体视觉。应用χ~2检验比较不同组别双眼远立体视重建情况,应用Logistic回归分析发病年龄、手术年龄、术前斜视度与双眼远立体视功能重建的相关性。结果:按照发病年龄分组,≤9岁组术后双眼远立体视重建率为38.67%,>9岁组为57.45%(χ~2=4.703,P=0.030)。按照手术年龄分组,18~28岁组术后双眼远立体视重建率为49.23%,29~39岁组60%,≥40岁组28.13%(χ~2=6.641,P=0.036)。按照斜视度分组,<50~△组术后双眼远立体视重建率为50%,50~89~△组42.86%,≥90~△组46.88%(χ~2=0.77,P=0.681)。发病年龄(OR=2.357,P=0.032)、手术年龄(OR=0.248,P=0.017)与成人外斜视术后双眼远立体视功能重建有相关性。结论:成人共同性外斜视发病年龄晚、手术年龄早的患者外斜视手术后恢复立体视几率大。Objective： To study the reestablishment of binocular distance stereopisis and the impact factors after adults concomitant exotropia surgery. Methods： The clinical data of 122 adults with concomitant exotropia surgery in our hospital were analyzed retrospectively. According to the age of onset, the age of operation and the preoperative strabismus degree, the patients were divided into different groups. Patients did not have simultaneous visual function before surgery. Binocular distance stereopisis was examined at 6 weeks after exotropia surgery. The Chi square test was used to compare the reestablishment of binocular distance stereopisis. Correlation between the age of onset, the age of operation, the preoperative strabismus degree and reestablishment of binocular distance stereopisis were analyzed by Logistic regression analysis. Results： According to the age of onset, the ratio of binocular stereo vision reconstruction in ≤9 years old group was 38.67%, 57.45% in 〉 9 years old group （X2=4.703, P=0.030）. According to the age of operation, the ratio of binocular stereo vision reconstruction in 18-28 years old group was 49.23%, 60% in 29-39 years old group, 28.13% in≥ 40 years old group （X2=6.641, P=0.036）. According to the strabismus degree, the ratio of binocular stereo vision reconstruction in 〈50A group was 50%, 42.86% in 50-89△ group, 46.88% in ≥90△ group （X2=0.77, P= 0.681）. The age of onset （OR=2.357, P=0.032） and the age of operation （OR=0.248, P=0.017） had correlation with the reestablishment of binocular distance stereopisis. Conclusion ： Late onset age, early age of concomitant exotropia surgery is more likely to restore stereopisis.
