子宫内膜癌合并多原发恶性肿瘤的临床病理特征

摘　　要：目的:探讨子宫内膜癌合并多原发恶性肿瘤(MPMN)患者的临床病理特点。方法:中日友好医院2003年1月~2015年3月接受手术治疗的子宫内膜癌患者共397例,回顾分析其中31例合并其他原发癌患者的临床资料。结果:子宫内膜癌合并多原发癌的发生率占同期子宫内膜癌的7.8%,子宫内膜癌中位发病年龄为50.3岁。原发癌的发生部位包括胃肠道、卵巢、乳腺、肺等。子宫内膜癌合并MPMN患者5年存活率为22.6%,存活时间中位数70±26.86个月,5年累积生存率55.8%,低于原发癌的5年生存率。原发癌间隔3年及以上者生存期较间隔3年以下者显著延长(P<0.05)。结论:子宫内膜癌合并MPMN器官多为结直肠、卵巢、乳腺、肺等,其5年存活率低,两原发癌发病间隔超过3年者生存期延长。Objective：To analyze the clinicpathological featuers of the multiple-primary-malignant neoplasms （MPMN） associated with endometrial carcinoma.Methods：A total of 31 patients suffered from MPMN among the 397 cases of endometrial carcinomas operated in our hospital from Jan.2003 to Mar.2015.Their clinico- pathological features were analyzed.Results：The incidence of MPMN was 7.8% among those with endometrial carcinoma.The median age of endometrial carcinomas patients was 50.3 years old.The major organs involved were colon,rectum,ovary,breast and lung.The 5-year survival rate of endometrial carcinomas with MPMN was 22.6%,the median survival time was 70＋26.86 months.The accumulate survival rate in 5 years of endometrial carcinomas with MPMN was 55.8%,was the lowest of any primary malignant neoplasm.The median interval between the first and the second primary neoplasm exceeding 3 years prolonged lifetime significantly than those less than 3 years （P〈0.05）.Conclusion：The major primary malignant neoplasms with in endometrial carcinoma are often happened in colon,rectum,ovary,breast and lung et al.The 5 year survival rate is lower than any primary neoplasms.The median interval between the first and the second primary neoplasm exceeding 3 years will prolong lifetime.
