腹腔镜联合内镜治疗105例结直肠肿瘤的疗效分析

摘　　要：目的探讨腹腔镜联合内镜治疗结直肠肿瘤的疗效及安全性。方法选取105例结直肠癌患者,按住院单双号分为2组,观察组(53例)行腹腔镜联合结肠镜切除手术,对照组(52例)行腹腔镜切除手术。观察并记录患者围手术期指标、近期疗效及随访2年期间并发症发生情况,以评价腹腔镜联合内镜治疗结直肠癌的疗效及安全性。结果2组在术后镇痛时间上相比,差异没有统计学意义(P>0.05)。观察组患者手术时间、术中失血量、住院时间均明显少于对照组(P<0.05);术后2组在下床时间、排气时间、排便时间上相比,差异没有统计学意义(P>0.05)。随访2年期间,观察组并发症共计5例,对照组9例,2组并发症发生率相比,差异没有统计学意义(P>0.05)。结论腹腔镜联合结肠镜手术,既可弥补腹腔镜手术时术者手触觉功能的丧失,又能弥补结肠镜手术时视觉功能的不足,明确肿瘤切除范围,缩短手术时间,减少术中出血量,患者恢复快,并且具有一定安全性。Objective To study the efficacy and safety of laparoscopic combined with endoscopy in the treatment of colo- rectal cancer. Methods 105 patients with colorectal cancer,were divided into 2 groups according to odd-even number of hospi- talization. The observation group（ 53 cases） were treated by laparoscopic combined with endoscopy. The control group（52 cases） were treated by laparoscopic surgery. The efficacy and safety oflaparoscopic combined with endoscopy in the treatment of colorectal cancer was evaluated by the perioperative indexes, short-term efficacy and complications during 2-year follow-up. Results There were no statistical significance in postoperative analgesia between the 2 groups （ P 〉 0.05 ）. The operative time, bleeding volume and hospitalization of the observation group was shorter or less than that of the control group（ P 〈 0.05 ）. There had no statistical significance in bed time, exhaust time and defecation time between the 2 groups. During 2-year follow-up, there were 5 cases in the observation group and 9 cases in the control group. There were no statistical significance in complications between the 2 groups（P 〉 0.05 ）. Conclusion Laparoscopic combined with colonoscopy can compensate for the loss of touching of the laparoscopic surgery, and compensate for visual problems of the colonoscopy. It can clear the extent of tumor resection, shorten operative time and decrease the bleeding volume, with quick recovery and safety.
