甲状腺良恶性肿瘤患者外科治疗后发生低钙血症的影响因素

摘　　要：目的:探究甲状腺良恶性肿瘤患者外科治疗后发生低钙血症的影响因素。方法:94例甲状腺良恶性肿瘤患者按低钙血症分为实验组(发生,52例)和对照组(未发生,42例),对两组患者多项指标进行统计并加以比较,采用logistics多因素回归分析研究影响低钙血症发生的影响因素。结果:两组患者年龄、性别组成、低白蛋白血症、高速酸血症、D-二聚体以及25-(OH)-D_3多项指标比较无统计学差异(P>0.05);实验组甲状腺双侧叶全切除比例、手术时间、二次手术比例、甲状腺癌比例、淋巴结全清扫比例、术后甲状旁腺素含量、降钙素含量与对照组相比差异显著(P<0.05);Logistics多因素回归分析结果显示手术范围大小、手术次数、甲状腺恶性程度、淋巴结清扫范围以及甲状旁腺素含量为低钙血症发生的独立危险因素,数据具有统计学意义(P<0.05)。结论:甲状腺双侧叶全切除、二次手术、甲状腺癌、淋巴结全清扫以及甲状旁腺素减少为术后低钙血症的独立危险因素。Objective：To explore influencing factors of hypocalcemia in treatment of thyroid benign and malignant tumor patients after surgery.Methods：94 cases of patients with benign and malignant thyroid tumors by hypocalcemia were divided into experimental group（occur,52 cases） and control group（not occur,42 cases）,the statistics of the two groups of patients with multiple indexes were compared,using logistics regression analysis to study the influencing factors on the influence of the incidence of hypocalcemia.Results：TTwo groups of age,gender composition,hypoalbuminemia,hypercholesterolemia,high acid two D-dimer and 25-（OH）-D3 had no significant difference（P〈0.05）;the experimental group total resection of thyroid bilateral leaf ratio,operation time,operation time,the proportion of two cases of thyroid carcinoma and lymph node dissection than the proportion,postoperative PTH and calcitonin content have significant difference with the control group（P〈0.05）;Logistics multivariate regression analysis showed that independent risk factors of operation range,operation times,thyroid malignant degree,lymph node dissection and parathyroid hormone content of hypocalcemia,statistically significant data（P〈0.05）.Conclusion：Thyroid lobe resection,bilateral two surgery,thyroid cancer,lymph node dissection and the parathyroid hormone reduce independent risk factors of postoperative hypocalcemia.
