摘　　要：目的:探讨原发性肝癌患者行肝动脉化疗栓塞术(TACE)前后中性粒细胞与淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)的变化情况,分析其影响因素。方法:回顾性分析2015年4月至2015年12月住院行TACE的原发性肝癌患者80例,观察患者术前、术后3天、术后2月的NLR变化情况。根据NLR水平进行分组,探讨不同NLR水平与无进展生存期的相关性。结果:术后3天NLR高于术前,差异有统计学意义(P<0.05),术后2月NLR下降至接近术前水平,差异无统计学意义(P>0.05)。术前患者高NLR水平主要与术前肿瘤BCLC分期、大小、AFP、血管侵犯有关。术后NLR高水平与术中明胶海绵使用情况有关。术前NLR高水平是影响预后的危险因素。结论:NLR可反应患者肝脏TACE后的炎症变化情况。术前NLR高水平是影响预后的重要指标。Objective：To investigate the changes of neutrophil-to-lymphocyte ratio （NLR） from patients with primary liver cancer （conventional） before and after hepatic artery chemoembolization,and analyze the influencing factors.Methods：Eighty cases of primary liver cancer patients were collected from April,2015 to December,2015,who were hospitalized in our department.We observed the changes of NLR level among three periods：Preoperation,three days after operation and 2 months after operation.We divided patients into groups according to NLR level,exploring the association among different NLR groups.Results：The level of NLR on 3 days after operation was higher than that before operation,the difference was statistically significant （P〈0.05）,while the level of NLR 2 months after operation fell close to preoperative levels,there was no statistically difference （P〉0.05）.The high preoperation-NLR level was mainly associated with BCLC installment,tumor size,AFP and vascular-invasion.The high postoperation-NLR level was associated with the gelatin sponge.The high preoperation-NLR leve was the risk factor which might affect the prognosis.Conclusion：The level of NLR can response changes of inflammatory of patients experienced TACE.The high postoperation-NLR level is one of the important prognostic factors for patients with TACE.
