摘 要:目的:观察光子照射治疗对乳腺癌改良根治术后并发症皮下积液和上肢淋巴水肿的疗效。方法:选取172例乳腺癌改良根治术后患者,按照随机数表法将其分为观察组(88例)和对照组(84例)。对照组术后第2 d创面开始采用常规创面清理及换药包扎治疗;观察组术后创面在与对照组相同治疗方法的基础上,采用光子治疗仪照射治疗。观察并比较两组患者创面术后第3d、第4 d及第5 d平均引流量,术后第5 d创面皮下积液和上肢淋巴水肿情况。结果:观察组创面术后第3 d、第4 d及第5 d平均引流量明显少于对照组,两组比较差异有统计学意义(t=-11.909,t=-13.481,t=-14.832;P<0.01);观察组术后第5 d创面皮下积液发生率明显低于对照组,两组比较差异有统计学意义(x^2=0.959,P<0.01);观察组术后第5d上肢淋巴水肿发生率低于对照组,两组比较差异有统计学意义(x^2=0.862,P<0.05)。结论:光子照射治疗可减少患者创面引流量,降低皮下积液和上肢淋巴水肿并发症发生率,促进创面愈合。Objective: To observe the therapeutic effect of photon irradiation therapy on postoperative complications(subcutaneous dropsy and upper limb lymph edema) in patients with modified radical operation of breast carcinoma. Methods: 172 cases of breast carcinoma patients with modified radical mastectomy were randomly divided into observation group(88 cases) and control group(84 cases). The control group was treated by routine cleaning and dressing on the wound at the second days after surgery, and the observation group was treated by the same treatment method combined with photon therapy. The average drainage volume of the surface of wound postoperative 3d, 4d and 5d between the two groups, respectively, were observed and compared. And the subcutaneous dropsy and upper limb lymph edema post-operative 5d between the two groups also were compared, respectively. Results: The average drainage volume of the observation group at 3d, 4d and 5d after operation, respectively, was less than the control group(t=-11.909, t=-13.481, t=-14.832; P〈0.01). Incidence rate of subcutaneous effusion was significantly lower than the control group at the fifth days(x^2=0.959, P〈0.01), and incidence rate of lymph edema also was significant lower than the control group at the fifth days(x^2=0.862, P〈0.05). Conclusion: Photon therapy can effectively reduce the drainage volume and the incidence of complication about subcutaneous effusion and upper limb lymph edema, and can promote wound healing.
