摘 要:目的探讨授权管理模式对提高急诊护理人员职业认同感与缓解职业倦怠的影响。方法针对急诊科患者疾病类型复杂多样、病情重、变化快、护理风险高的特点,仁和医院于2015年1—6月期间在急诊科实施护理风险授权管理模式,将科室内22名护理人员分为5个风险管理小组,每个小组由4名护理人员组成,由各小组分别完成相应的护理风险控制。管理时间为6个月,采用护理质量、护理人员职业倦怠及职业认同感来评价授权管理效果。结果实施授权管理模式后,急诊室基础护理、急救知识及技能、急救器械及药品管理、院感控制、护理文书书写质量评分均明显高于授权管理前(t=4.992~6.670,P<0.05);急诊室护理人员职业认同感、职业目标、职业态度、职业价值观、职业自我概念、总分明显高于管理前(t=4.172~7.268,P<0.05);情感耗竭、去人格化、低个人成就感得分、工作倦怠量表(MBI)总分均明显低于授权管理前(t=3.757~5.828,P<0.05)。结论授权管理模式有助于提高急诊护理质量,可能与增强护理人员职业认同感、缓解职业倦怠有关。[Objective]To investigate the influence of authorization management pattern on improving professional self-identity and relieving occupational burnout of emergency nursing personnel. [Methods]In view of the characteristics of patients in emergency department,such as complex and varied disease types,severe illness state,quick change and high nursing risk,the authorization management pattern of nursing risk was implemented in the emergency department of Renhe Hospital from January to June in 2015. 22 nurses were divided into five risk management groups, 4 nurses in each group, and groups separately completed their corresponding nursing risk control. The management duration was 6 months and the effect of authorization management was assessed by the nursing quality, occupation burnout and professional self-identity of the nursing personnel. [Results] After the implementation of the authorization management pattern,the scores of basic nursing,first aid knowledge and skills,first aid equipment and pharmaceutical administration,hospital infection control and writing quality of nursing document in the emergency department were significantly higher than those before the authorization management(t=4.992-6.670,P〈0.05). The total score of professional self-identity,career objectives,professional attitude,occupational values and vocational self-concept were significantly higher than those before the authorization management(t =4.172-7.268,P〈0.05). The total scores of emotional exhaustion,depersonalization,sense of low personal achievements and Maslach Burnout Inventory(MBI) were significantly lower than those before the authorization management(t=3.757-5.828,P〈0.05).[Conclusion]The authorization management pattern is conductive to improve the quality of emergency nursing,which may be related to the improvement of the professional self-identity and the remission of the occupational burnout of the nursing personnel.
