糖尿病合并低血糖的危害与救治

摘　　要：低血糖是糖尿病患者治疗过程中最常见的急性并发症之一，严重影响糖尿病患者的血糖达标和治疗进程。低血糖给糖尿病患者带来的损害是多方面的，包括严重的心血管事件、认知功能障碍甚至痴呆、增加住院重症患者死亡率等。指南及专家共识建议通过加强患者的管理和教育、自我血糖监测（SMBG）、个体化治疗等手段减少低血糖的发生。Objective To study the dynamic change of stromal cell-derived tactor 1 （ SDF-1 ） level of acute ischemic stroke （AIS） in peripheral blood and the correlation of SDF-1 level with the outcome of AIS. Methods A total of 92 AIS patients and 20 patients with risk factors were recruited. SDF-1 in the plasma was measured by enzyme linked immunosorbent assay（ELISA）. Functional recovery was assessed at the 90th day after AIS by modified Rankin Scale （ mRS ）. AIS patients were divided into good prognosis group（71 cases） and poor prognosis group （21 cases）. Compare and analyze the SDF-1 levels between two groups. Results Compared with the Risk group,SDF-1 level in patients with AIS on the first day of admission and on the 7th day of illness were both higher. The level of SDF-1 on the 7th day of illness was significantly higher than that of the first day of admission, and the differences were statistically different（ P 〈 0.01 ）. Within AIS patients, compared with the poor prognosis group, SDF-1 level of good prognosis group on the 7th day of illness rose（ P 〈 0.05 ） ,which was significantly higher than that of the first day（P 〈0.01 ）. The change rate of SDF-1 was correlated with the mRS scores on the 90th day after AIS （ t = 1. 211, P = 0. 044）. Conclusion There exist dynamic changes of SDF-1 level in peripheral blood of patients with AIS, which are related to the mRS scores of 90th day after AIS. SDF-1 level could be used as the predictor of the prognosis of AIS.
