摘 要:目的:探讨牙周炎对冠心病患者血管钙化的影响。方法:选取诊断为冠心病及冠心病合并中、重度牙周炎患者共150例,依据冠状动脉造影结果将其管腔狭窄程度分为轻(A组)、中(B组)、重(C组)三组,各组均为50例,三组分别包括冠心病患者(A1组、B1组、C1组)及冠心病合并中、重度牙周炎患者(A2组、B2组、C2组)各25例。计算和分析所有患者冠状动脉钙化积分(coronary artery calcium score,CACS)值、血脂及C-反应蛋白(CRP)水平的含量,并且对牙周炎患者相关牙周临床指数进行对比分析。结果:在三组患者中冠心病合并中、重度牙周炎患者较单纯冠心病患者总CACS值高(P<0.05);A2、B2、C2组分别较A1、B1、C1组血脂及CRP水平升高,差异具有统计学意义(P<0.05);A2、B2、C2三组间的牙周临床指数有差异(P<0.05),且随着管腔狭窄程度加重,PD和CAL值均呈显著升高。结论:牙周炎对冠心病患者血管钙化有一定的促进作用,同时冠心病也在一定程度上加重对牙周组织的破坏。Objective:To explore the influence of periodontitis on vascular calcification related to coronary heart dis -eases .Methods:One hundred and fifty patients diagnosed with coronary heart disease or complicated by moderate and severe periodontitis were collected.They were divided into mild(group A),moderate(group B)or severe(group C)groups based on the luminal stenosis degree according to the coronary arteriography with 50 patients in each group ,and each group had 25 pa-tients(group A1,groupB1,group C1)with coronary heart disease and 25 patients(group A2,groupB2,group C2)with coronary heart disease complicated by moderate and severe periodontitis .The values of coronary artery calcium score ( CACS ) , blood lipid and C-reactive protein(CRP)level of all patients were calculated and analyzed ,and the clinical index in patients with periodontitis were compared .Results:Among three groups ,the coronary heart disease patients with moderate to severe peri-odontitis had a higher CACS than the patients with simple coronary heart disease (P〈0.05).Blood fat and CRP level of ser-um in the patients with periodontitis (A2,B2,C2)were higher than those in patients without periodontitis (A1,B1,C1),and the difference had statistical significance (P〈0.05).The periodontal clinical indices were different among group A 2,B2 and C2.At the same time,PD and CAL were significantly increased with the increase of coronary artery stenosis .Conclusion:Pe-riodontitis may contribute to the vascular calcification of the coronary heart disease patients .Meanwhile,coronary heart dis-ease can increase the destruction of periodontal tissue to some extent .
