摘 要:目的探讨320排CT双入口灌注技术容积成像在肺部占位性病变诊断中的价值。方法前瞻性分析2012年10月至2016年5月我院收治肺部占位病变120例,所有病例均经组织病理学明确诊断,其中恶性病变88例,腺癌62例,鳞癌20例,小细胞癌5例,淋巴瘤1例;良性病变32例,其中慢性炎症26例,肺结核6例。所有病例均行320排CT双入口灌注扫描得到肺部病变的灌注参数:肺动脉血流量(PF),支气管动脉血流量(BF),灌注指数[PI=PF/(PF+BF)]。利用后处理软件对数据进行灌注分析。结果本组120例中88例恶性病变的支气管动脉血流量BF高于良性病变,肺动脉血流量PF及灌注指数PI值低于良性病变,良、恶性病变的肺动脉血流量(PF)、支气管动脉血流量(BF)及灌注指数(PI值)的差异均具有统计学意义(P<0.05)。以PI值≤56.30作为恶性病变阈值,88例恶性病变78例确诊,符合率88.6%,32例良性病变28例确诊,符合率87.5%。结论 CT双入口灌注成像可以提供肺部占位病变的微血管信息,其PI值可以对肺部良恶性病变的鉴别诊断提供帮助。Objective To investigate the application value of 320 slice CT dual-input volume perfusion technique in the differentiation of lung occupying lesions.Methods A total of 120 patients with lung occupying lesions admitted into the hospital during October 2012 to May 2015 were prospectively analyzed.All cases were diagnosed by pathology.Among the objects,88 cases were malignant lesions,including adenocarcinoma(62cases),squamous cell carcinoma(20cases),small cell carcinoma(5cases)and lymphoma(one case).32 cases were benign lesions including chronic inflammatory lesions(26cases)and pulmonary tuberculosis(6cases).All cases underwent 320 slice CT dual-input volume perfusion scanning and the blood flow perfusion parameters of lesions:pulmonary flow(PF),bronchial flow(BF)and perfusion index[PI=PF/(PF+BF)]were gotten.The dual-input perfusion data were transmitted to the workstation and were analyzed by post-processing soft.Results In this group,the BF of malignant lesions was higher than that of benign lesions and the PF and PI were lower than that of benign lesions.The differences of PF,BF and PI values between benign and malignant lesions were statistically significant(P〈0.05).If the PI value which was equal to or less than55.60 was defined as the threshold of malignant lesions,there were 78/88 cases malignant lesions were diagnosed and the coincidence rate was 88.6%,26/32 cases benign lesions were diagnosed and the coincidence rate was 87.50%.Conclusion 320 slice CT perfusion imaging can reflect the microvascular characteristics of different types of lung lesions.The PI value is helpful to the differentiation of benign and malignant lung lesions.
