微小染色体维持蛋白7在鼻腔鼻窦鳞状细胞癌中的表达及临床意义的研究

摘　　要：目的检测微小染色体维持蛋白7(MCM7)在鼻腔鼻窦鳞状细胞癌(SNSCC)中的表达水平,分析其与肿瘤组织分化程度及患者预后的关系。方法采用免疫组化法检测SNSCC和慢性鼻窦炎鼻息肉(NPCRS)中MCM7的表达情况,并分析其与临床病理特征的关系。结果 NPCRS中,MCM7阳性细胞主要分布于上皮基底层,且表达率较低。SNSCC中,MCM7阳性细胞呈弥漫性分布,且表达率较高。MCM7在SNSCC中的表达水平高于NPCRS,差异有统计学意义(P<0.001)。MCM7的表达仅与肿瘤分化程度有关(P=0.001),且随着分化程度的下降而逐渐增高。SNSCC患者总体3年和5年生存率分别为61.3%和46.0%。Ⅰ~Ⅲ期和Ⅳ期患者的3年生存率分别为90.0%和25.6%,5年生存率分别为70.0%和17.1%,差异有统计学意义(P<0.001)。MCM7阴性患者3年、5年生存率为36.0%和18.0%,而阳性患者3年、5年生存率为59.9%和34.2%,差异无统计学意义(P=0.297)。结论MCM7在SNSCC中表达水平显著升高,而且与肿瘤分化程度有关,随着肿瘤分化程度下降,MCM7表达水平逐渐增高,但与SNSCC患者的生存率无关。Objective To investigate the expression of minichromosome maintenance protein 7（MCM7）in sinonasal squamous cell carcinoma（SNSCC）,and evaluate its relationship with tumor differentiation and prognosis of patients. Methods Using immunohistochemistry,MCM7 expression in SNSCC and nasal polyps with chronic sinusitis（NPCRS）were studied,and relationships between markers and clinicapathological features were analyzed. Results In NPCRS,MCM7 positive cells were mainly distributed in the epithelial basal layer and the expression rate was low,whereas in SNSCC,MCM7 positive cells were diffuse and the expression rate was high. MCM7 expression was significantly higher in SNSCC than in NPCRS（P 〈 0.001）and related to tumor differentiation（P = 0.001）,increasing gradually with decreasing degree of differentiation. The overall 3-and 5-year survival rates of patients with SNSCC were 61.3% and 46%,respectively.The 3-year survival rates for patients with stage Ⅰand Ⅳ were 90% and 25.6%,respectively,and the 5-year survival rates were 70% and 17.1%,respectively;the difference was statistically significant（P 〈 0.001）. The 3-and 5-year survival rates of MCM7-negative patients were 36.0% and 18.0%,respectively,and those of MCM7-positive patients were 59.9% and 34.2%,respectively;the difference was not statistically significant（P = 0.297）. Conclusion In SNSCC,MCM7 expression is significantly increased,inversely associated with the degree of tumor differentiation,and unrelated to the survival rates of patients.
