感染评分及小野寺指数在老年结直肠癌术后感染及预后评估中的应用价值

摘　　要：目的探讨感染评分及小野寺指数在老年结直肠癌术后感染及预后评估中的应用价值,为术后感染的防治及预后评估提供参考依据。方法选取2013年1月-8月在医院行手术治疗的老年结直肠癌患者163例,术前计算感染评分及小野寺指数;观察患者术后感染情况,追踪随访患者生存率。结果术后感染患者37例,感染率为22.70%,以呼吸道感染为主,共17例占45.94%;患者术后1年时随访到患者152例,生存132例,生存率86.84%;3年时随访到患者102例,生存64例,生存率为62.75%;术前小野寺指数营养良好与营养不良患者术后1年生存率分别为95.74%、72.41%,3年生存率分别为79.03%、37.50%,比较差异有统计学意义(P<0.05)。结论感染评分高及小野寺指数值低的老年结直肠癌术后感染风险明显增高,感染评分对术后生存时间无影响,小野寺指数值用于患者预后评估有一定价值。OBJECTIVE To discuss evaluation values of infection probability score and Onodera′s prognostic nutrition index in postoperative infection and prognostic evaluation of elderly patients with colorectal cancer,so as to provide a reference for postoperative infection prevention and treatment and prognosis evaluation.METHODS A total 163 cases of elderly patients with colorectal cancer underwent surgical treatment from Jan.2013 to Aug.2013 were selected.The preoperative infection probability score and Onodera′s prognostic nutrition index were calculated,the postoperative infection and follow-up of patients survival rate were observed.RESULTS Totally 37 cases were postoperatively infected,with the infection rate of 22.70%,which were mainly 17 cases of respiratory infection,accounting for 45.94%.Totally 152 patients had one-year follow-up,and 132 cases survived,with the survival rate of 86.84%.Totally 102 patients had three-year follow-up,and 64 cases survived,with the survival rate of 62.75%.The one-year survival rates were 95.74% and 72.41%in patients with good nutrition good and poor nutrition of Onodera′s prognostic nutrition index,the three-year survival rates were 79.03% and 37.50%,and the differences were significant（P〈0.05）.CONCLUSIONPostoperative infection rate is higher in elderly patients with colorectal cancer with high infection probability score and low Onodera′s prognostic nutrition index,the infection probability score has no effect on the postoperative survival time,and Onodera＇s prognostic nutrition index value has a certain value for patients prognostic evaluation.
