摘  要:目的 探讨依达拉奉对原发性肺癌微创外科患者氧化应激与炎症因子影响的研究.方法 选择2014年1月至2016年1月就诊的原发性肺癌微创外科患者48例,随机分为对照组和观察组,每组24例.所有患者双腔支气管插管并机械通气,对照组在通气5 min前静脉滴注氯化钠注射液,观察组静脉滴注依达拉奉(1 mg/kg).比较两组患者的一般临床资料和手术相关指标,测定两组患者在手术前麻醉后以及术后2 h时炎症因子分肺损伤指标水平.结果 两组年龄、手术相关指标包括单肺通气时间、手术时间、术中失血量以及液体总量、手术前后最大压力(Pmax)和呼气末二氧化碳分压(PetCO2)比较差异未见统计学意义(P〉0.05);手术前两组炎症因子和TNF-α水平比较差异未见统计学意义(P〉0.05),手术后观察组白细胞介素(IL-6)、IL-8、IL-10和肿瘤坏死因子(TNF-α)水平明显低于对照组(P〈0.05);手术前两组的肺表面活性物质相关蛋白A(SP-A)、肺表面活性物质相关蛋白D(SP-D)、丙二醛(MDA)和超氧化物歧化酶(SOD)水平比较差异未见统计学意义(P〉0.05),观察组有效降低了SP-A、SP-D和MDA水平(P〈0.05),观察组有效提高了SOD水平(P〈0.05).结论 依达拉奉治疗原发性肺癌微创外科患者,可降低IL-6、IL-8、IL-10以及TNF-α的水平,降低MDA水平和升高SOD水平,同时对降低肺损伤指标SP-A、SP-D指标的含量水平,对肺具有保护作用,值得临床推荐.Objective To study the effect of edaravone on stress oxidation and inflammatory factor in primary lung cancer patients with minimally invasive surgery.Methods The 48 patients with primary lung cancer who received minimally invasive surgery from January 2014 to January 2016 were randomly divided into control group (n=24) and observation group (n=24).All patients with double lumen endotracheal intubation and mechanical ventilation, control group with infusion of sodium chloride injection 5 min before ventilation, the observation group with intravenous infusion of edaravone (1 mg/kg).The clinical data of the two groups were compared and the related indexes were measured.The levels of inflammatory cytokines in the patients of the two groups before and after the operation were measured at the time of 2 h after operation.Results There was no significant difference in age, surgery related inderes including single lung ventilation time, operation time, amount of bleeding during surgery or total amount of liquid, the maximum pressure (Pmax) and PetCO2(P〉0.05);Before surgery, there was no significant difference in inflammatory cytokines or tumor necrosis factor alpha (TNF-α) level (P〉0.05) between the two groups, the levels of of interleukin (IL)-6, IL-8, IL-10 and TNF-α in the observation group were significantly lower than those in the control group after operation (P〈0.05);Before surgery, there was no significant difference in pulmonary surfactant associated protein A (SP-A), pulmonary surfactant associated protein D (SP-D), malondialdehyde (MDA) or superoxide dismutase (SOD) level (P〉0.05), the SP-A, SP-D and MDA levels reduced, and soplevel increased in observation group (P〈0.05).Conclusions Edaravone in treatment of patients with primary lung cancer who underwent minimally invasive surgery, can reduce the levels of IL-6, IL-8, IL-10 and TNF-α, decrease the level of MDA and increase the level of SOD, at the same time reduce the levels of SP-A, SP
