生大黄经鼻空肠灌入与血液净化治疗重症急性胰腺炎临床观察

摘　　要：目的 探讨生大黄经鼻空肠灌入联合血液净化治疗重症急性胰腺炎（SAP）的临床效果.方法 选取2011年1月至2015年12月SAP患者75例,随机分为两组.对照组36例给予常规综合治疗及血液净化治疗.观察组39例早期加用生大黄经鼻空肠灌入.观察两组治疗效果.结果 治疗7 d后,观察组有效率、中转手术率、病死率分别为82.1%（32/39）、5.2%（2/39）、12.8%（5/39）,对照组分别为61.1%（22/36）、22.2%（8/36）、16.7%（6/36）,两组有效率比较差异有统计学意义（χ^2=4.0717,P=0.0436）.治疗后,观察组指标均优于对照组,差异有统计学意义（P〈0.05）.结论 生大黄鼻空肠灌入联合血液净化治疗SAP安全有效,能提高救治成功率.Objective To investigate the efficacy of rhubarb by nasal feeding and blood purification on severe acute pancreatitis （SAP）. Method Seventy-five patients with SAP were selected from January 2011 to December, and randomly divided into the two groups. A total of 36 cases was treated with routine treatment and blood purification therapy as control group, 39 cases in observation group were treated with rhubarb nasal jejunal infusion on this basis. Effect was compared between the two groups. Results Seven days after treatment, efficient, transit operation rate, fatality rate of observation group were 82. 1% （32/39）, 5.2% （2/39）, 12. 8% （5/39）, which of the control group were 61.1% （22/36）, 22.2% （8/36） , 16.7% （6/36） , the efficient had significant difference between the two groups （χ^2 = 4. 0717, P = 4. 0717）. Indicators of observation group were better than those of control group, the differ- ences were significant （ P 〈 0. 05 ）. Conclusions Rhubarb nasal feeding combined with blood purifica- tion in treatment of SAP is safe and effective, and can improve the success rate of treatment.
