医护一体化模式在骨科护理中的应用研究

摘　　要：目的探讨医护一体化模式在骨科护理中的应用研究。方法选择2013年2月—2016年6月绍兴市人民医院收治拟行骨科手术患者120例,按照随机数字表法分为对照组和观察组,各60例。2组患者均由同一组医护人员完成手术。对照组患者给予常规护理方式,观察组患者给予医护一体化护理模式。采用视觉模拟评分(VAS)评估2组患者术后不同时间点疼痛度,观察2组患者术后恢复情况及术后并发症情况,评价2组患者护理满意度。结果观察组患者VAS评分在术后24、48、72h均显著低于对照组患者,差异具有统计学意义(P<0.05),观察组患者术后下床时间、术后进食时间、住院时间均短于对照组,差异均具有统计学意义(P<0.05),观察2组患者术后并发症发生率,对照组发生10例便秘、3例下肢深静脉血栓、3例肺炎、4例泌尿系感染,共20例并发症;观察组发生6例便秘、1例下肢深静脉血栓、1例肺炎、2例泌尿系感染,共10例并发症,观察组术后并发症发生率显著低于对照组,差异具有统计学意义(P<0.05),观察组患者总满意度为100.00%,显著高于对照组患者的83.33%(Z=3.422,P<0.05)。结论采用医护一体化模式对减轻骨科患者术后疼痛,减少术后并发症,缩短术后恢复时间,提高护理满意度,因此值得临床推广应用。Objective To explore the application of medical and nursing integration model in orthopedic nursing. Methods Total 120 patients planning orthopedic surgery in our hospital from February,2013 to June,2016 were enrolled and divided into control group and observation group according to the random number table method （ n = 60）. The operations for all patients were performed by the same group of medical staffs. The patients in the control group were given routine nursing care and the patients in the observation group were given the integrated nursing care mode. The visual analogue scale（VAS） was used to evaluate the postoperative pain degree of the two groups. The postoperative recovery and postop- erative complications of the two groups were observed. The patient satisfaction with nursing care in the two groups was e- valuated. Results The VAS score of the observation group was significantly lower than that of the control group at each time point（ P 〈 0.05 ）. The postoperative bedtime, postoperative feeding time and hospital stay were shorter in the observa- tion group as compared with the control group（ P 〈 0.05 ）. In the control group, there were 10 eases of constipation ,3 eases of deep venous thrombosis in lower extremity, 3 cases of pneumonia, and 4 cases of urinary system infection ; in the observation group, there were 6 cases of constipation, 1 case of deep venous thrombosis in lower extremity, 1 ease of pneumo- nia, and 2 cases of urinary system infection, the incidence of postoperative complications in the observation group was sig- nificantly lower than that in the control group. The total patient satisfaction with nursing care in the observation group was 100%, which was obviously higher than that （83.33%） in the control group （ Z = 3. 422, P 〈 0.05 ）.
