胃癌中基质金属蛋白酶、人组织激肽释放酶7和E-钙粘蛋白的表达及其临床意义

摘　　要：目的:探讨血清中MMPS、KLK7、E-cad在胃癌患者的表达与临床病理参数间的关系,并探讨二者相关性。方法:胃癌患者50例以及50例胃良性疾患者和50例健康体检者,分别命名为实验组、胃部良性疾患组和健康体检组。实验组中有24例符合手术条件同意行胃切除术的患者,细分为术前组和术后组。采用酶联免疫吸附试验(ELISA)检测胃癌患者术前术后(1月)、胃良性疾患者组、健康体检组血清中MMP-2、MMP-9、KLK7及E-cad水平,分析与各临床病理指标的相关性。结果:实验组血清KLK7及E-cad水平高于胃良性疾患组及正常体检组,差异均显著(P<0.05);实验组术后术前比较,MMP-2、MMP-9均较术前减少,KLK7及E-cad术后分别明显下降和升高,差异显著(P<0.05);实验组Ⅰ~Ⅳ期血清KLK7水平及E-cad水平随癌症的进展分别呈现升高和下降趋势,单因素方差分析结果显示,P<0.05;实验组中胃癌有无微血管转移与血清KLK7及E-cad表达水平有关(P均<0.05)。实验组的年龄分组、肿瘤大小、分化程度、浸润深度等不同病理特征的血清KLK7及E-cad水平的比较显示,KLK7及E-cad表达水平差异无统计学意义(P>0.05)。胃癌患者血清KLK7及E-cad水平表达呈显著负相关(r=-0.386,P<0.01)。结论:KLK7和E-cad的表达上调或者下调都参与了胃癌的发生发展和转移过程。两者在胃癌患者血清中表达呈负相关,在胃癌发展及转移中可能存在拮抗效应。联合检测KLK7与E-cad可能对监测胃癌患者的病情及估计预后有一定帮助。Objective：To research the expression level of klk7 and E-cad in serum in patients with gastric cancer and to explore the relationship of klk7 and E-cad with gastric cancer clinical pathological characteristics.Method：50 cases of patients with gastric cancer and 50 cases of benign gastric disease patients and 50 healthy persons were named as the experimental group,benign gastric disease group and healthy group.The experimental group,24 patients met the conditions agreed to surgery underwent gastrectomy were divided into preoperative groups and postoperative groups.The serums levels of KLK7 and E-cad were detected by enzyme-linked immunosorbent assay（ELISA）.Results：the serum KLK7 and E-cad level is higher than that of benign gastric disease group and normal control group,the difference was significant（P〈0.05）;compared with the experimental group after operation,MMP-2 and MMP-9 were decreased,KLK7 and E-cad respectively after operation was significantly decreased and increased significantly（P〈0.05）;the experimental group I-in the level of serum KLK7 and E-cad levels with stage IV cancer were increased and decreased,the single factor variance analysis showed that P〈0.05 of gastric cancer;in the experimental group without microvascular invasion and serum KLK7 and the expression level of E-cad,P〈0.05.The comparison of serum KLK7 and E-cad levels in different age groups,tumor size,differentiation degree and depth of infiltration in the experimental group showed that there was no significant difference in the expression of KLK7 and E-cad between the two groups（P〈0.05）.Serum KLK7 and E-cad expression in gastric cancer patients were significantly negatively correlated（r=-0.386,P〈0.01）.Results：the serum KLK7 and E-cad level is higher than that of benign gastric disease group and normal control group,the difference was significant（P〈0.05）;compared with the experimental group after operation,MMP-2 and MMP-9 were decreased,KLK7 and E-cad respectively after operation was significant
