摘 要:研究Thl7细胞在急性髓系白血病(AML)患者骨髓与外周血中的表达水平,进一部探讨急性髓系白血病的发病机制。并探讨Thl7细胞比例的改变与病人临床的相关性。方法①用实时荧光定量(RT-PCR)分别检测AML初诊组、化疗完全缓解及正常对照组骨髓及外周血单个核细胞中Th17转录因子(RORC)的表达水平。②用流式细胞术分别检测AML初诊组、化疗完全缓解及正常对照组外周血中Thl7细胞所占比例。结果 AML初诊组外周血中Th17细胞表达水平(0.98±0.058)明显低于完全缓解组(2.08±0.76)及正常对照组(1.79±0.306),差异有统计学意义(P<0.05)。AML初诊组(中位数0.000 036 418 3范围0.000 003 328 5~0.001 410 087 1)外周血单个核细胞(PBMCs)RORC m RNA的表达水平明显低于完全缓解组(中位数0.000 187 543 4范围0.000 025 426 0~0.001 221 892 7)及正常对照组(中位数0.002 644 026范围0.000 063 627 0~0.000 956 465 1),差异有统计学意义(P<0.001)。初诊组(中位数0.000 036 7983范围0.000 005 010 3~0.000 512 556 9)骨髓单个核(BMMCs)RORC,显著低于完全缓解组(中位数0.000 102 885 9范围0.000 014 073 4~0.001 568 208 7),差异有统计学意义(P<0.001)。初诊的AML患者接受治疗达到完全缓解后外周血RORC m RNA的表达恢复到正常水平,差异有统计学意义。结论 Thl7细胞表达失衡可能与AML的发生发展相关。Objective To research the expression level of Thl7 cell in bone marrow and peripheral blood of AML patients and further study the pathogenesis pf acute myeloid leukemia and correlation between Thl7 cell ratio change and patients clinical manifestations. Methods The expression level of RORC in bone marrow and peripheral blood in the AML first-visit group, chemotherapy complete remission group and normal control group were tested by the RT-PCR. The Thl7 cell ratio in peripheral blood in the AML first-visit group, chemotherapy complete remission group and normal control group was tested by the flow cytometry. Resul ts The Th17 cell expression level in the peripheral blood in the AML first-visit group was ob-viously lower than that in the chemotherapy complete remission group and normal control group, [(0.98±0.058) vs (2.08土 0.76), (1.79±0.306)], and the difference had statistical significance( P〈0.05), the (PBMCs) RORC mRNA expression level in the AML first-visit group ( median: 0.000 036 418 3, range; 0.000 003 328 5?0.001 410 087 1), was obviously lower than that in the complete remission group,( median: 0.000 187 543 4, range: 0.000 025 426 0-0.001 221 892 7) and in the nor-mal control group, (medians: 0.002 644 026, range: 0.000 063 627 0-0.000 956 465 1), and the difference had statistical significance, P〈0.001, and the(BMMCs) RORC in the first-visit group ( medians: 0.000 036 798 3, range: 0.000 005 010 3- 0.000 512 556 9) was obviously lower that in the complete remission group, ( medians: 0.000 102 885 9, range: 0.000 014 073 4-0.001 568 208 7), and the difference had statistical significance ( P〈0.001), and the RORC mRNA expression re-turned to the normal level after treatment in the AML first-visit group with statistical significance.
