两种不同开髓方法对下颌恒切牙根管预备的影响及效果比较

摘　　要：目的比较唇侧开髓和舌侧开髓对下颌恒切牙根管预备的影响及应用效果。方法选取2014年1月至2015年12月收治的下颌恒切牙需要行根管治疗的患者105例共173颗牙作为研究对象,根据开髓方式不同分为试验组(55例患者88颗牙)和对照组(50例患者85颗牙)。对照组采取舌侧开髓的方式,试验组采取唇侧开髓的方式,对比两组的应用效果及玷污层分级情况。结果试验组治疗1周后有效率为88.6%,明显高于对照组的70.6%,1年后的治愈率为89.8%,明显高于对照组的72.9%,差异均有统计学意义(P<0.05)。两组下颌恒切牙牙冠1/3的玷污层分级比较差异有统计学意义(P<0.05)。结论下颌恒切牙唇侧开髓的方式对根管的效果优于舌侧开髓,具有一定的临床价值。Objective To provide a reference for mandibular permanent incisors by comparing the effects of mandibular open pulp and lin-gual open pulp on mandibular root canal preparation. Methods From January 2014 to December 2015 in our hospital were admitted to Department of Stomatology mandibular incisor need root canal therapy in 105 patients with a total of 173 teeth as the research object, according to the pulp were divided into experimental group and control group, the control group took the lingual pulp to open the way, the experimental group adopted the labial endodontic way the application effect of the two groups were compared and smear layer classification. Results After 1 week treatment, the effective rate of the experimental group was significantly higher than that of the control group, and the cure rate of the experimental group was significantly higher than that of the control group after one year, and the difference was statistically significant （P 〈0. 05） . Statistically significant differences be-tween the two groups of patients with smear layer classification of mandibular incisor crown comparison of 1/3 （P 〈0. 05） . Conclusion The way of opening the pulp of the mandibular permanent incisors is better than the lingual open pulp.
