白内障手术对糖尿病患者泪液影响的研究

摘　　要：目的探讨白内障超声乳化吸除联合人工晶状体植入手术对糖尿病患者泪液的影响。方法选择30例单纯性老年性白内障患者和30例患有糖尿病的白内障患者,分别为对照组和糖尿病组。在术后4个时间点7 d、15 d、1个月和3个月进行干眼症状评分,光学相干断层扫描(OCT)分析泪河高度,泪膜破裂时间(break-up time,BUT)和SchirmnerⅠ结果测定,对2组上述指标进行分析。结果术后7 d、15 d及1个月糖尿病组干眼症状评分分数较高(P<0.05);术后3个月对照组较糖尿病组干眼症症状评分差异无统计学意义(P>0.05)。术后7 d、15 d、1个月及3个月对照组较糖尿病组泪河线高度数值高,BUT时间长,差异有统计学意义(P<0.05或<0.01)。术后15 d、1个月及3个月糖尿病组SchirmnerⅠ长度较对照组明显减少,差异有统计学意义(P<0.01)。结论糖尿病组较对照组白内障超声乳化吸出联合人工晶体植入术后干眼症状重,且持续时间更长。Objective To investigate the effects of phacoemulsification combined with artificial lens implantation on tear fluid of patients with diabetes mellitus（ DM）. Methods Thirty patients with simple senile cataract and 30 cataract patients with DM were divided into control group（ n = 30） and DM group（ n = 30）. The symptoms scroes of xerophthalmia were performed on 7d,15 d,1m,3m after the surgery,more over,the OCT was used to analyze lacrimal riuer height,tear fluid break-up time（ break-up time,BUT） and Schirmner Ⅰ measurement results. The detection results were analyzed and compared statistically between two groups. Results The symptoms scroes of dry eye in DM group were higher than those in control group on 7d,15 d,1m after surgery,however,there were no significant differences in the scores on 3m after surgery between two groups. The lacrimal riuer height and BUT on 7d,15 d,1m,3m after surger in control group were higher than those in DM group,further more,Schirmner Ⅰ lenth in DM group was significantly decreased on 15 d,1m,3m after surgery.Conclusion The syndromes of xerophthalmia in patients with DM are more severe and duration is more longer than those in patients with simple senile cataract after phacoemulsification.
