血清游离轻链在多发性骨髓瘤中的临床诊断研究

摘　　要：目的探讨血清游离轻链(s FLC)在多发性骨髓瘤(MM)中的临床诊断价值。方法收集初诊MM患者96例作为研究组,另收取同时期进行体检的健康人群45例作为对照组。采用免疫比浊法检测s FLC-κ及s FLC-λ,并计算s FLC-κ/λ比值。结果对96例MM患者使用IFE进行M蛋白分型,结果显示,Ig G型患者人数最多。κ-MM组患者s FLC-κ、s FLC-κ/λ水平明显高于对照组,λ-MM组患者s FLC-κ、s FLC-λ水平明显高于对照组,s FLC-κ/λ明显低于对照组,差异有统计学意义(P<0.05)。s FLC-κ/λ比率高患者与低比率患者ISS国际分期存在明显差别(P<0.05)。结论s FLC的检测对于MM的临床诊断有积极的意义,s FLC-κ/λ与MM分期具有相关性。Objective To explore the clinical value of serum free light chains （sFLC） in the diagnosis of multiple myeloma. Methods 96 cases of multiple myeloma were selected as the study group and 45 healthy patients underwent physical exam were the control group. Then the sFLC-κ,sFLC-λ 2 were detected by immunoturbidimetry, and sFLC-κ/λ were calculated. Results 96 cases of multiple myeloma used immunofixation electrophoresis to M proteotyping, the M protein results showed that the number of IgG patients was the highest. The sFLC-K and sFLC-κ/λ of K-MM group were obviously higher than the control group. The sFLC-K, sFLC-κ of κ-MM group were obviously higher than the control group, and the sFLC-κ/λ of λ-MM group was lower than control group. The differences had statistically significance （ P 〈 0.05 ）. There had statistically differences in ISS stage between patients with different sFLC-κ/λ rate （ P 〈 0.05 ）. Conclusion sFLC detection has positive significance in the detection of multiple myeloma, sFLC-κ/λ rate has correlation with multiple myeloma stage.
