摘 要:目的探讨多层螺旋CT应用于周围型肺癌TNM分期诊断中的意义。方法选取110例周围型肺癌患者,先行X线片检查,后进行多层螺旋CT检查。观察并记录2种方法肺癌阳性检出率,周围型肺癌临床TNM分期的诊断、检查时间及费用,以评价多层螺旋CT对周围型肺癌TNM分期诊断的意义。结果多层螺旋CT对肺内病灶和肺外病灶的阳性检出率均明显高于X线(P<0.05)。根据外科病理TNM分期结果,多层螺旋CT对周围型肺癌的临床TNM分期诊断符合率为90.9%,X线对周围型肺癌的临床TNM分期诊断符合率为61.8%,多层螺旋CT对周围型肺癌的临床TNM分期诊断符合率明显高于X线(P<0.05),但X线所需检查时间及费用均明显低于或少于多层螺旋CT(P<0.05)。结论多层螺旋CT对于周围型肺癌具有较高的检出率,可以较好地进行临床TNM分期,但费用较高,检测时间较长。为降低周围型肺癌的漏诊率和误诊率,可以将X线和多层螺旋CT结合。Objective To study the value of multi-slice CT in the diagnosis of TNM stage of peripheral lung cancer patients. Methods 110 patients with peripheral lung cancer were selected, they were divided into 2 groups according to different examination methods. The X-ray group ( 110 cases) was performed with X-ray examination, and the CT group ( 110 cases ) was examined by multi-slice CT. The significance of muhi-slice CT in the diagnosis of peripheral lung cancer patients with clinical TNM stage was evaluated by positive rate of lung cancer, the diagnosis of peripheral lung cancer with clinical TNM stage, examination time and cost. Results The positive rate of multi-slice CT in pulmonary and extrapulmonary lesions was higher than that of the X-ray( P 〈 0.05 ). According to the surgical pathologic TNM stage, the coincidence rate of multi-slice CT in the diagnosis of peripheral lung cancer with clinical TNM stage was 90.9% , the coincidence rate of multi-slice CT in the diagnosis of peripheral lung cancer with clinical TNM stage was 61.8%. So the coincidence rate of multi-slice CT in the diagnosis of peripheral lung cancer with clinical TNM stage was higher than that of the X-ray( P 〈 0.05 ). The checking time and cost of X-ray was lower or less than that of the multi-slice CT( P 〈 0.05 ). Conclusion Multi-slice CT had a high detection rate for peripheral lung cancer, which could be used to detect clinical TNM stage. But the cost is high and detection time was long. In order to reduce the miss rate and misdiagnosis rate of peripheral lung cancer. The multi-slice CT could combine with X-ray to improve the level of clinical diagnosis.
