白内障超声乳化术联合房角分离术治疗原发性闭角型青光眼

摘　　要：目的分析白内障超声乳化术联合房角分离术治疗原发性闭角型青光眼(PACG)的临床应用疗效.方法选取2014年1月至2016年6月玉溪市人民医院收治的94例原发性闭角型青光眼并发白内障患者作为研究对象,按照随机数字表将其分为对照组(n=47)及实验组(n=47),对照组采用超声乳化联合小梁切除术治疗,实验组采用超声乳化术联合房角分离术治疗,对比2组临床治疗前后的眼压、视力、房角粘连度的变化.结果术后3 d、1个月、3个月、6个月实验组的眼压为(13.6±4.4)mm Hg、(14.1±4.3)mm Hg、(13.3±4.2)mm Hg、(15.4±4.7)mm Hg,对照组的眼压分别为(17.6±4.4)mm Hg、(18.7±5.3)mm Hg、(19.4±5.8)mm Hg、(18.2±5.6)mm Hg,较术前均明显降低(P<0.05),实验组与对照组眼压同期对比,下降显著(P<0.05);在视力恢复、粘连度下降并发症方面,实验组均好于对照组(P<0.05).结论在PACG治疗方面,与超声乳化术联合小梁切除术相比,采用超声乳化术联合房角分离术治疗效果较好,不仅操作简单创伤小,安全性高,而且能够显著降低眼压,提高患者视力,开放房角,加深前房,值得临床推广应用.Objective To analyze the clinical effect of phacoemulsification combined with goniosynechialysis in primary angle closure glaucoma. Methods Ninety-four cases of primary angle closure glaucoma complicated with cataract patients from January 2014 to June 2016 in Yuxi People＇s Hospital were selected as the research object, and were randomly divided into control group （n=47） and experimental group （n=47） . The control group was given phacoemulsification combined with trabeculectomy for treatment, and the experimental group was given phacoemulsification combined with goniosynechialysis treatment. The changes in the angle of adhesion degree, the intraocular pressure and visual acuity of patients between 2 groups were compared before and after treatment. Results Postoperative IOP 3D, 1 months, 3 months and 6 months in the experimental group was （13.6 ± 4.4） mmHg, （14.1 ±4.3） mmHg, （13.3 ±4.2） mmHg, （15.4 ±4.7） mmHg control group, the intraocular pressure were （17.6 ± 4.4） mmHg, （18.7 ± 5.3） mmHg （19.4 ± 5.8） mmHg, （18.2 ± 5.6） mmHg, were significantly lower than that before operation （P〈0.05） . Compared with control group at the same period, the intraocular pressure of patients in the experimental group decreased more significantly （P〈0.05） In the recovery of visual acuity, the decrease of adhesion degree and the incidence of complications, the experimental group was better than the control group （P〈0.05） . Conclusion In the treatment of PACG, compared with phacoemulsification combined with trabeculectomy, phacoemulsification combined with goniosynechialysis treatment has better effect, simpler operation, less trauma, higher safety, and this treatment can significantly reduce IOP and improve visual acuity, open angle, deepen the anterior chamber, and is worthy of promotion.
