联合泪道引流管植入治疗泪小管断裂123例的疗效分析

摘　　要：目的观察联合泪道引流管植入治疗外份性泪小管断裂吻合术后疗效和特点。方法回顾分析2014年1月至2016年4月我科收治的外伤性泪小管断裂患者123例123眼,其中所有患者均在显微镜下寻找到泪小管断端,分别由上、下泪小点插入泪道引流管,沿泪小管走行进人断端至鼻泪管,并顺利实施吻合手术。对术后反应、术后效果及并发症等情况进行回顾性分析。结果 123例吻合成功患者中,有效66例(53.66%),好转41例(33.33%),无效16例(13.01%),总有效率为86.99%,随访6个月,泪道冲洗均通畅。术后并发症:所有患者中眼睑外翻4例(325%),泪小点撕裂1例(0.81%)。结论泪小管吻合联合泪道引流管植入可用于治疗外伤性泪小管断裂,均可获得良好的效果,术后反应轻,术后并发症少,值得临床推广。Objective To observe the curative effect and characteristics of combined lacrimal drainage tube implantation for the treatment of traumatic lacrimal canalicular anastomosis. Methods 123 cases of 123 patients with traumatic lacrimal canal rupture were retrospectively analyzed from January 2014 to June 2016.All patients under the microscope to find the lacrimal canal stump respectively, from the upper and lower lacrimal puneta inserted into the lacrimal drainage tube, along the canal into the end of the nasolacrimal duct, and the successful implementation of anastomosis. Postoperative response, postoperative results and complications were retrospectively analyzed. Results Of the 123 cases of successful anastomosis, 66 cases （53.66%） were effective, 41 cases improved （33.33%）, 16 cases （13.01%） were ineffective. The total effective rate was 86.99%. Postoperative complications 4 cases （3.25%） of eyelid ectropion and 1 case （0.81%） of tears punc- tal tear in all patients. Conclusions Lacrimal duct anastomosis combined with lacrimal drainage tube can be used for the treatment of traumatic lacrimal canal rupture. All of them can obtain good results. The postoperative reaction is less and the postoperative complications are less.
