改良泪囊鼻腔吻合术治疗复发性慢性泪囊炎的临床效果

摘　　要：目的探讨改良泪囊鼻腔吻合术治疗复发性慢性泪囊炎的临床效果。方法将2012年7月~2016年2月在我院确诊为首次泪囊鼻腔吻合术后失败或术后复发的6个月以上的80例慢性泪囊炎患者纳入研究并随机分为对照组和观察组,各40例。对照组采用传统鼻腔泪囊吻合术,观察组采用改良泪囊鼻腔吻合术,比较两组的临床疗效。结果两组患者均顺利完成手术,观察组的术中出血量、住院费用更少,手术时间、住院时间更短,差异有统计学意义(P<0.05)。观察组术后1周、1个月、3个月、6个月及12个月时的溢泪评分显著低于对照组,差异有统计学意义(P<0.05)。结论改良泪囊鼻腔吻合术治疗复发性慢性泪囊炎效果显著,具有创伤小,恢复快的优势,优于传统鼻腔泪囊吻合术,值得推广。Objective To explore the clinical efficacy of modified dacryocystorhinostomy in the treatment of recurrent chronic dacryocystitis.Methods A total of 80 patients who were definitely considered as the failure of dacryocystorhinostomy for the first time or postoperative recurrence for more than 6 months from July 2012 to February 2016 in our hospital were selected and randomly divided into the control group and the observation group,40 cases in each group.The control group was treated with traditional nasal dacryocystorhinostomy,the observation group was treated with modified dacryocystorhinostomy.The clinical efficacy was compared between the two groups.Results Surgeries in the two groups were successful.The intraoperative amount of bleeding and hospitalization expense were less and operation time and hospital stay were shorter in the observation group compared with those in the control group,with significant differences （P〈0.05）.The scores of tear overflowing at 1 week,1 month,3 months,6 months and 12 months after surgeries in the observation group was significantly lower than that in the control group,with significant differences （P〈0.05）.Conclusion The modified dacryocystorhinostomy is effective in the treatment of recurrent chronic dacryocystitis,at advantages of small trauma and quick recovery,which is superior to the traditional nasal daeryocystorhinostomy and is worthy of promotion.
